Personal Choice Service Advisor

Full-time (40 Hours per week) 

General Responsibility: Service Advisors will provide home and community-based services to elders and disabled adults throughout the State of Rhode Island. Service Advisors serve as an integral member of the Personal Choice Program Team. Service Advisors will assist in providing person-centered care in coordination with Mobility Specialists and Nurses. Service Advisors will assist in the goal of providing support and services to enable seniors and disabled adults to maintain their independence and highest quality of life through the provision of appropriate home and community-based services, including supports for family caregivers.

Specific Responsibilities:Provide beneficiaries with the opportunity to exercise choice and control over their personal care. Assist clients in making informed decisions that are consistent with their needs and individual circumstances. Assess clients for initial eligibility for the program and to determine/identify the needs of the client. Develop and write an Individual Service and Spending Plan. Assist in determining the amount of assistance needed. Reassess clients on an annual basis. Assist in identifying and removing barriers to improve independence. Assist in developing, implementing and monitoring Personal Choice services. Provide training and assistance to clients and their representatives. Maintain contact with the client via monthly phone calls. Conduct at least two face-to face home visits with each client (scheduled annual, 6 month and/or unannounced). Complete and submit critical incident reports as necessary to EOHHS. Report any suspicions of Medicaid fraud to their immediate supervisor and take appropriate action as necessary. Coordinate completion of all forms with client’s medical provider(s). Coordinate completion of client application with the fiduciary agent (Options), EOHHS, OMR and DHS. Complete Medicaid application with client. Complete re-certifications with client. Coordinate service delivery with Tri-County’s nurse and mobility specialist. Participate in client appeals process. Continued training of Personal Choice Program Service Advisors. Conduct monthly internal audits of at least ten (10) Personal Choice Program participant charts. After each monthly audit, a summary will be written and kept in a binder with each audit sheet. Responsible for writing internal policies and procedures for the Personal Choice Program, as needed. Liaise between Tri-County Community Action Agency Service Advisors and EOHHS personnel. Attend EOHHS/DEA/NHP supervisors meetings at the discretion of the Senior and Disabled Adult Services Director. Ensure all necessary forms/paperwork are kept up-to-date, according to program standards. Be present for, and if necessary, assist with, audits from EOHHS and/or Neighborhood Health Plan of Rhode Island. Coordinate annual client satisfaction survey. All other duties as assigned by the Personal Choice Program Team Lead and/or Senior and Disabled Adult Services Director.

Qualifications: Bachelor’s Degree in Social Work, Gerontology, Sociology or closely related field required. Must have at least two years of case management experience working with the elder population.

Special Qualifications: Must have a valid driver’s license with valid vehicle insurance. Must be able to pass a Criminal Background Check (state and national). Must have working knowledge of the provision of health care in a variety of settings, knowledge of community resources, insurance and care delivery systems as well as health and mental health interventions. Possess the skills and techniques for crisis intervention and problem solving. Possess effective verbal, written and active communication skills. Demonstrate the ability to be culturally competent and ability to create and sustain trusting relationships with clients and their families. Ability to analyze situations and make immediate decisions.
