Tri County Community Action Agency is Hiring a Nurse Practitioner Near Johnston, RI

Nurse Practitioner

Full-time (40 hours per week)

General Responsibility: Tri-County Community Action Agency is currently seeking full-time Nurse Practitioners to provide primary care services in our Federally Qualified Health Center (FQHC). Primary Care Providers (PCP) will work collaboratively with other team members in providing high-quality, comprehensive primary medical care to Tri-County patients using best practice and evidence-based standards. Tri-County is an NCQA recognized Level III Patient Centered Medical Home and as part of the team-based approach to care, the PCP will participate in coordinating care for patients, support patients and families in self-management and will engage in continuous quality improvement activities aimed at specific patient population needs. We are particularly seeking individuals with a strong commitment to community health and working with the medical underserved. We particularly seek providers with a commitment to our mission “To provide accessible, quality, and comprehensive social, educational, health, prevention, and other services to people in need, while empowering them to achieve the highest possible level of self-sufficiency and quality of life.” 

Qualifications: Graduate of an accredited master’s level Nurse Practitioner program; Current RI NPP License, Board Certified/Board eligible in Family Practice. DEA license.  Prior experience practicing in Patient Centered Medical Home (PCMH) preferred. Understand and practice
