Tri County Community Action Agency is Hiring a Case Manager-Adult Protective Services Near Providence, RI

Full-time (40 Hour) position serving Northern County 

General Responsibility: Case manager is responsible for investigating suspected complaints of elder abuse, neglect, exploitation, or self-neglect in R.I. for persons age 60 and older. Tri-County contracts with The Office of Healthy Aging to provide ongoing case management services for such older adults who are in need of Adult Protective Services.

 Specific Responsibilities:Conduct un-announced in home client assessments based on established RI OHA timeframes, care coordination, advocacy and related case management activities for assigned clients. Develop individualized plan of care and recommended services that address all pertinent areas of the assessment. Work collaboratively with client, client’s family, medical personnel, ancillary health care providers and community service providers. Work collaboratively with identified elder law and fire department advocates, first responders, housing and building code enforcement and residential service coordinators. Provide follow up to the client following the initial assessment for a minimum of three months to ensure continuity of care. Assist elders in gaining access to community health and support services. Coordination of care with RI OHA to ensure that client’s needs are addressed in an integrated manner. Collect and report information regarding the client in standardized format as dictated by RI OHA. Assume responsibility for all timely data entry as related to the elder. Comply with all Office of Healthy Aging and Tri-County Community Action Agency policies and procedures and with all current state and federal laws and regulations related to confidential client information and records.

 Qualifications: Bachelor’s Degree in Social Work, Sociology, Gerontology, Human Services or related field required. At least two years of case management experience preferably with the elder population. 

Special Qualifications: Possess strong organizational, verbal and written skills as well as strong computer skills with Microsoft Office products. Working knowledge of the provision of health care in a variety of settings. Familiarity with pertinent elder abuse dynamics and the legal system. Knowledge of community resources. Ability to communicate and work collaboratively with elders, families and community resources. Ability to work with various cultures. Knowledge of social, health, and mental health interventions. Ability to analyze situations, problem solve, and make immediate decisions in crisis situations. Ability to create and sustain trusting relationships with clients and families and practice active listening skills. Maintain an active driver’s license, maintain automobile insurance and have access to reliable transportation. Be available to travel throughout the state. Pass a state and national criminal background check. Demonstrate an understanding of everyone’s experiences, every day.
