As an In-Home Hospice Registered Nurse (RN) with Tri-Cities Chaplaincy, you will play a vital role in delivering high-quality hospice care in the comfort of patients' homes. Your responsibilities encompass providing skilled nursing care, pain management, and emotional support to patients facing end-of-life challenges. You will work collaboratively with a compassionate team of healthcare professionals to ensure patients receive the best possible care and comfort during their journey.

At Tri-Cities Chaplaincy, we carefully establish salary ranges for our positions by taking various factors into account, including education, certifications, knowledge, skills, experience, and market conditions. These ranges are tailored to match the diverse programs and services we offer. Our goal is to ensure fair compensation for candidates based on these factors while maintaining internal peer equity.

Pay Range:$35.00 - $73.81/hour

Starting Pay Range for this Position: $35.50/hr - $65.79/hr

· Mileage Reimbursement: $0.67 per mile, for eligible positions.

· Vacation: Accrual of hours each pay period based on hours worked.

· Wellness Days: After the 90-day introductory period, eligible employees can convert up to two sick days into wellness days.

· Annual "Yay Day": Full-time employees receive 1 paid day off every January to volunteer in the community.

· Sick Time: Earned on an accrual basis, with a 90-day hold for new hires (usable after 90 days)

· Paid Holidays: Ten recognized holidays with no waiting period for new hires.

· Health Benefits: Opt-in for Low Deductible Health Benefits, Vision, and Dental starting the first month following your start date.

· Health Benefit Options: Medical, Dental, Vision, Supplemental Life, Accidental Death and Dismemberment, GlobeLife Voluntary Benefits, and Life Flight.

· Voluntary Supplemental Benefits: Pet Insurance, Globe Life, and Life Flight. 

· Agency-covered benefits: Basic Life Insurance and Accidental Death and Dismemberment at 1x Annual Salary, Long-Term Disability, and Short-Term Disability.

· 401k: You can start contributing right away and up to 5% employer matching after 1 year of employment for eligible employees.

· Premium Pay: Bilingual in Spanish: Verified through ALTA staff testing and Approved Certifications: Premium pay upon review and approval.

· Tuition Assistance: After 1-year of employment, eligible employees can receive up to $5,000 annually in higher education assistance.

· Continuing Education Units: Tri-Cities chaplaincy utilizes Relias Learning and CAPC for employees to earn continuing education units for their credential requirements.

Certification and Training Opportunities: Employees can partner with their department director on professional goals that benefit the agency and themselves for certification and training opportunities 

Requirements

MINIMUM POSITION QUALIFICATIONS

  1. Associate Degree in Nursing.
  2. Unrestricted Registered Nurse Licensure in Washington State
  3. Excellent verbal and written communication.
  4. Excellent critical thinking and problem-solving skills.
  5. Moderate computer skills.
  6. Ability to work independently and as part of a team.
  7. Available to work on-call a minimum of two-holiday shifts per year.
  8. This position is designated as safety-sensitive and will require pre-employment drug testing, which includes cannabis testing in WA State.

PREFERRED POSITION QUALIFICATIONS

  1. Bachelor’s Degree in Nursing.
  2. Bilingual in Spanish.

AGENCY REQUIREMENTS

  1. Demonstrate support and adherence to the agency’s Mission, Vision, and Values.
  2. Responsible for knowledge and compliance with agency policies, procedures, and practices.
  3. Responsible for maintaining and protecting Patient and agency information in a confidential manner. Practice appropriate HIPAA confidentiality by accessing patient-related information on a “need to know” basis only.
  4. Reliable personal transportation, current and valid driver’s license, and vehicle insurance.
  5. Satisfaction with state and agency requirements for Tuberculin testing 
  6. Hepatitis B immunization series, proof of series, or signed declination.
  7. Proof of applicable Vaccination(s) on file or approved accommodation/declination as appropriate on file.
  8. Certification in Cardio-Pulmonary Resuscitation (CPR)/Basic Life Safety (BLS)
  9. Must pass criminal background screening and pre-employment drug screening.
  10. Ability to meet the physical demands of the position (see attached physical requirements and environmental conditions).
  11. Complete agency and departmental orientation and training program(s).
  12. Complete all continued learning, education, policy reviews, and required training by their respective deadlines.
  13. Maintain licensure(s) and certification(s) required for the position.
  14. Available to work as assigned/scheduled. Anticipated schedule is four 10-hour shifts and includes every other weekend. 

RESPONSIBILITIES AND ESSENTIAL FUNCTIONS

  1. Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process.
  2. Provide skilled nursing assessment, planning, and care to maximize the comfort and health needs identified by the patient/family unit.
  3. Coordinates services to provide preventive and supportive care to the terminally ill and their families in accordance with approved care plans and Hospice policies and procedures.
  4. Directs the work of the Licensed Professional Nurse and Certified Nursing Assistant as assigned.
  5. Ensure compliance with IDG meetings at a minimum every 15 days and as needed.
  6. Patient care documentation entered into EMR within 24 hours.
  7. Patient care plan entered into EMR prior to Nursing Assistant assignment.
  8. Regularly attend scheduled staff meetings and in-services.
  9. Update facility staff on changes to care plans and maintain regular communication with administration staff at facilities of assigned patients.
  10. Utilize and order resources such as medical supplies, medications, and DME appropriately.
  11. Initiate communication with attending physician(s), other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
  12. Maintain regular communication with the Clinical Nursing Supervisor to review caseload.
  13. Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.
  14. Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
  15. Seek input from other team members regarding the patient’s plan of care to obtain additional knowledge and support.
  16. Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur.
  17. Instruct primary caregivers, volunteers, and employed caregivers to provide care as indicated.
  18. Meet regularly with the interdisciplinary team members to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth.
  19. Attending patient care conferences and facilitating discussion of issues from caseload for full staff discussion, consultation, and evaluation.

20. Supervise and document observance of the Certified Nursing Assistant’s delivery of care every two weeks, and actual on-site observation every two months.

21. Inform the Clinical Nursing Supervisor of unusual, or potentially problematic, patient/family issues.

22. Assist other registered nurses including assuming primary responsibility for patients, when necessary.

23. Provide appropriate support at the time of death and perform bereavement assessment if requested.

24. Participate in agency and community programs as requested to promote professional growth and understanding of hospice care.

25. Cross-train in other nursing areas as part of orientation and as needed/assigned.

26. Availability to cover after-hours on-call shifts as needed and as assigned. 

27. Provide coverage or “float” to other nursing areas in the agency as needed and as assigned, particularly during the orientation period when patient caseload needs may be low.

28. Perform other duties as assigned.
