We are seeking a Housing Coordinator to help support the work we do.

Job Function:

The Housing Coordinator assists people living with HIV who are experiencing housing instability to navigate their housing options. This includes creating housing plans with clients and linking them to resources. Under the supervision of the Community Health Program Manager, the Housing Coordinator works the Community Health team and other Trellus staff to ensure the full circle of services to the clients and the community.

Responsibilities:

• Conduct intake assessments to assess needs and develop housing service plans for new and existing clients
• Implement project scopes of services and ensure compliance to program policies.
• Produce and submit monthly, quarterly, and annual reports, as required, in a timely manner.
• Maintain and update housing resources, client databases, files, and required documentation.
• Provide confidential and culturally sensitive services in working with clients and gathering information for client files and housing applications.
• Assist in the development, organization, and implementation of workshops for clients.
• Attend required team meetings and program planning meetings.
• Meet program related training requirements.
• Adhere to agency missions and policies
• Other duties as assigned

Qualifications (Skills, Knowledge, & Experience):

• Bachelor’s degree or equivalent experience in social work with experience in housing assistance.
• Experience working with HIV/AIDS clients preferred. Compassion and desire to work with diverse and complex clients.
• Bilingual and/or experience working with HIV population or housing placement is preferred.
• Able to drive, with valid Illinois driver’s license and body/liability insurance; or use public transportation when needed.
• Excellent writing, speaking, and organizational skills.
• Ability to quickly establish rapport with clients and handle interpersonal situations professionally.
• Excellent observational skills and attention to detail
• Ability to multi-task and prioritize.
• Accurately document case notes and enter data under tight deadlines.
• Work independently and in a team
• Proficiency with Microsoft Office and other related applications
