POSITION DESCRIPTION

Supervisor: Quality Assurance Program Manager
Location: East Portland Office - Hybrid
Hours: Monday – Friday; 9:00am-5:00pm, some weekend and evening work required
Wage: $23.50 Hourly

GENERAL RESPONSIBILITIES
Assist in administrative documentation of case management services and programming in a manner that is consistent with Transition Projects mission, values, policies, and procedures. Participate actively in planning, delivery, and documentation of services. Function as grant compliance support to ensure accuracy and completeness of files and data. This is a non-exempt, represented full-time position.

SPECIFIC RESPONSIBILITIES
- Maintain accurate, updated and comprehensive participant files.
- Assist in the maintenance of accurate and updated documentation of services in HMIS.
- Assist with HMIS correction projects as needed.
- Conduct file reviews in order to monitor file and data completeness and accuracy.
- Develop and maintain relations with housing case managers and agency staff to coordinate file fixes and provide consistent quality and comprehensive services to participants.
- Request and review rental assistance for program participants through Kintone, using HMIS and case notes to record expenses.
- Complete monthly data corrections and assist in the training of staff as it related to data completeness and file compliance.
- Provide support and assist staff in following case management best practices and documentation requirements.
- Attend staff meetings, case management meetings and partner meetings as assigned.
- Adhere to allocated budgetary guidelines.
- Provide monthly Case Management Reports as requested.
- Follow Case Management Best Practices.
- Other duties as assigned.
PERFORMANCE REQUIREMENTS
Each Transition Projects employee must:

- Be tested for TB within 30 days of hire and be retested at least annually thereafter throughout employment.

- Acquire First Aid/CPR/AED certification within 30 days of hire and be retested biannually thereafter throughout employment.
- Complete Bloodborne Pathogens and Narcan Training within 30 days of hire and be retested annually thereafter throughout employment.
- Demonstrate effective communication skills by conveying necessary information accurately, listening effectively and asking questions when clarification is needed.
- Plan and organize work effectively and ensure its completion.
- Exercise necessary cost control measures.
- Demonstrate reliability by being present for work as scheduled, arriving and leaving on time and taking breaks in expected time frames.
- Interact effectively with persons of diverse ethnic backgrounds, religious views, cultural backgrounds, and sexual orientations and treat each individual with respect and dignity.
- Embrace teamwork and promote a team-oriented environment.
- Demonstrate initiative.
- Make decisions that reflect consistency with Transition Projects philosophy, policies, and procedures.
- Participate in ongoing development and improvement of all program processes and relationships.
- Attend agency and program staff meetings.
PHYSICAL/MENTAL REQUIREMENTS
The physical and mental demands described here are representative of those that must be met by an employee, with or without accommodation, to successfully perform the essential functions of this job.
- Hearing and vision adequate for interaction with participants, interaction with staff and interaction with the general public.
- Hearing adequate for telephone work. Vision adequate for close work.
- Hand and finger dexterity adequate to operate standard office equipment.
- Ability to walk, bend, stand, sit, reach, stoop, pull, squat, and climb stairs.
- Sitting at a desk completing paperwork and working on a computer for lengthy periods.
- Mobility to allow responsive traveling to participant residences located around the metro area.
- Ability to perform CPR for fifteen minutes continuously.
MINIMUM QUALIFICATIONS
The right individual for this position has a passion for helping others and a desire to work in an organization that serves people who are experiencing poverty and homelessness.
- Applicants must possess a bachelor’s degree. Degree related to social work or administration preferred.
- Demonstrated ability to interact with other agencies in developing and providing services to participants; including ability to advocate on behalf of participants for services with other agencies.
- Ability to identify and work effectively with property and building managers, developing positive long-term relationships.
- Ability to provide mediation between agencies, participants, and property and building managers.
- Up-to-date familiarity of community resources and services available to assist the homeless.
- Demonstrated knowledge of domestic violence and sexual assault issues and resources.
- Demonstrated knowledge of services available to assist veterans.
- Demonstrated ability to work as part of a team in the delivery of services to participants.
- Computer literacy. Demonstrated ability to use word processing, database, spreadsheet, e-mail, and Internet programs. Basic knowledge of Excel is required.
- Demonstrated ability to maintain accurate and comprehensive participant files.
- Demonstrated ability to prepare accurate and comprehensive statistical and narrative reports.
- Bilingual English/Spanish preferred. Applicants who are themselves veterans will receive priority consideration.
