Responsibilities

• Coordination of care with entire care team
• Completing home care nursing visits
• Educating patients and family members
• Preparing timely, complete and accurate clinical notes
• Communicating with healthcare providers and other care team members

Qualifications

• LVN / LPN in the State of Texas
• Home Health experience a must
• Knowledge of Home Health regulations and general healthcare compliance
• (Preferred but not required) Experience with (HCHB) Home Care Home Base EMR
• Truly cares for people and will go above and beyond to provide outstanding patient care
