NOTES: Hospital Care at Home program
Duties:
Facilitates patient screening and enrollment into the Hospital Care at Home program by working with ED and inpatient providers to proactively identify patients. Run reports in the EMR multiple times/day to identify patients that may be eligible, reviewing medical team documentation and documenting findings in the EMR appropriately.
Lead multidisciplinary daily huddles to discuss potential patients and available capacity. Communicate action items and ensure team members follow up so patients can be transferred home in a timely manner.
Verifies patient eligibility for inclusion, including clinical, social and payor requirements are met prior to enrolling patients. Coordinates as needed with internal stakeholders.
Introduces program to patient, answers questions, and obtains patient consent. Follows up with patients and families/caregivers (where appropriate) throughout the patient consent and enrollment process.
Administers post-discharge phone call surveys and interviews patients about their experience in Hospital Care at Home.
