Touch of Care Hospice, a family-owned and operated agency, is seeking a compassionate and dedicated Hospice Registered Nurse to join our rapidly growing team in the Mesa County Area. We are committed to providing exceptional care and support to patients and their families during one of life's most challenging times.

As a Hospice Registered Nurse, you will be responsible for delivering high-quality, compassionate care to terminally ill patients and their families. You will work closely with our interdisciplinary team to assess patient needs, develop individualized care plans, and provide skilled nursing care in accordance with established standards and regulations.

- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs

- Develop and implement personalized care plans in collaboration with the interdisciplinary team

- Provide skilled nursing care, including medication management, wound care, and symptom management

- Educate patients and families on disease progression, treatment options, and end-of-life care

- Coordinate care with other healthcare providers and community resources

- Maintain accurate and up-to-date patient records and documentation

- Provide emotional support and counseling to patients and families

**Qualifications:**

- Valid Registered Nurse (RN) license in the state of Colorado

- Minimum of 1 year of nursing experience, preferably in hospice or palliative care

- Excellent communication and interpersonal skills

- Compassionate and empathetic approach to patient care

- Strong critical thinking and problem-solving abilities

- Ability to work effectively in a fast-paced environment and collaborate with a multidisciplinary team
