Core Competencies

Takes and/or assists in taking referrals as needed, in cooperation with a licensed nurse. Answers phones/faxes and receives all intake information including authorizations and enters the appropriate information in the computer. Completes intake screens for pending admission. Schedules appropriately skilled person to fill the case; inputs scheduling information on the computer. Schedules any replacement workers needed; inputs scheduling information on the computer. Takes telephone referral information if no nursing staff are available and passes referral on to nurse as soon as possible. Handles all matters related to these personnel and the client they serve, with the exception of issues that require professional/clinical judgment. Assists in maintenance of adequate staffing patterns and time-off records for staff. Coordinates the scheduling of all field staff employees. Maintains case management notes that documents communication that relates to client care and how each situation was handled. Communicates on a routine basis with contract agencies to maintain smooth relations and to obtain needed information. Checks cases after filling and weekly thereafter. Transfers field staff from one case to another, where appropriate. Prepares weekly schedule for field staff and copies for distribution. Assists in coordinating services provided to clients. Controls and monitors schedule changes Makes copies of referral sheets when requested. Checks compliance of visits done (on master schedule); brings and scheduling problems to the supervisor’s attention immediately. Attends case conferences as requested by his/her supervisor. Assists in relaying messages to field staff, office staff, and community liaisons. Maintains current client roster with necessary information. Assists in data entry as requested. Obtains insurance authorizations and/or eligibility when necessary from physician office.
• Reviews utilization to ensure authorizations are obtained timely for those services that require prior authorization.
