TN Voices is Hiring a HERO Case Manager- Adult Near Goodlettsville, TN

Qualifications: Case Manager provides coordination and consultation for the physical and mental health care of adult clients. Valid driver’s license, proof of automobile insurance, and reliable transportation. Training in proper crisis prevention initiatives and universal precautions required. A bachelor’s degree in a Behavioral health field or closely related field with one (1) or more years of experience in the field. The Case Manager is focused on the whole health of our clients with appropriate referrals and collaboration with team members and specialists for conditions requiring monitoring or medical intervention.

Skills & Knowledge Required:

  • Ability to evaluate, monitor and support clients’ well-being and stability in various life domains.
  • Knowledge of the developmental, mental, emotional, relational, and social needs of adults.
  • Knowledge of behavioral disorders and adult mental health diagnoses, including DSM V.
  • Knowledge of the service area and of resources for adults with mental health issues.
  • Must have good communication skills and a natural ability to establish rapport with adults as well as work collaboratively as a member of a team.
  • Must possess the ability and desire to learn new skills to include cultural competency, special education policies, procedures, and rights, addressing the needs of transition age youth.
  • Effective self-care and independent living strategies.
  • Strength-based interventions and positive reinforcement.
  • Ability to gather information to make informed recommendations and referrals to appropriate resources.
  • Ability to help clients identify their strengths and needs.
  • Ability to assist with the development, implementation and monitoring of individualized treatment goals.
  • Ability to manage a complex caseload of high-need, chronically mentally ill clients.
  • Ability to travel, access to reliable transportation and current auto insurance required.
  • Ability to meet with clients served in home and in the community.
  • Excellent organizational, written, communication and problem-solving/creative thinking skills.
  • Access to reliable home internet is required.

Duties & Responsibilities/Principal Activities:

  • Meet face to face with each client a minimum of once per month to assess general welfare and ensure for the provision of all personal and treatment-related needs.
  • Provide on-call crisis support up to and including response by phone or in person at the client’s home and/or in the community.
  • Complete required documentation within established timeframes.
  • Assist client with scheduling, transportation and advocacy for all community-based services and treatment.
  • Participate in weekly team meetings and case reviews.
  • Present case updates including progress and barriers during Case Reviews and be willing to contribute feedback and direction to other team members.
  • Ensure appropriate documentation is completed in a timely manner (e.g. consent, release of information, progress notes, etc.)
  • Responsible for the development and implementation of an individualized strength-based plan of care for each client in collaboration with the client, program peers and other treating professionals.
  • Respond to crisis in a prompt, effective and collaborative manner.
  • Uses a framework of promotion, prevention, and intervention to promote mental health, resilience and prevent challenging behaviors.
  • Connects client and providers to community resources to support social, emotional and mental health development.
  • Provides evidence-based tools, resources and activities which support the client in social and emotional development, behavior, and mental health and wellness.
  • Assist clients with monthly budgeting of money and resources.
  • Assist in creating and implementing interventions and strategies to address the client vocational issues.
  • Assist the clinical team in creating and implementing interventions and strategies to address the client’s substance abuse issues if applicable.
  • Assist the clinical team in creating and implementing interventions that build positive affect between the client and their natural supports and professional service providers.
  • Model and teach effective self-advocacy skills, in keeping with the system of care principles.
  • Complete a client-centered care plan to ensure all needs of the client are met.
  • Work collaboratively with team members to ensure mental, physical and social needs are met for the client.
  • Conform to CARF standards as applicable.
  • Participate in individual supervision, reflective supervision groups, agency staff meetings, and trainings.
  • Submit timely and accurate documentation of services, time sheets, and other required paperwork.
  • Comply with all TNV Policies and Procedures.
  • Comply with Federal Confidentiality Law to protect the rights of the consumer.
  • All other duties as appropriate and as needed for the success of assigned clients not specifically denoted within this Job description.
