Plans, implements and evaluates patient care plans to restore or maintain patient well being. Performs clinical assessment to identify patient needs and appropriateness of patient. 2. Instruct on use of equipment such as walkers, wheelchair, canes, braces, prosthetic devises and appliances in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs. Orders appropriate rehabilitative equipment to meet patients' needs and adapts equipment, appliances to meet home limitations/requirements.5. Provides appropriate patient/family education regarding therapeutic procedures based on the patient's age, condition, culture and learning readiness.6. Performs and oversees other team members, when applicable, in the performance of patient care that is consistent with patient's needs and desires.7. Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.8. Accepts clinical assignments that are consistent with education and competence to care for patients.9. Performs other related duties as assigned or requested.Knowledge/Skills/Abilities:1. Uses effective interpersonal relations and communication skills.2. Knowledge of durable medical equipment.3. Meets mandatory continuing education requirements.4.
