We are seeking skilled Registered Nurses (RNs) to join our team as a Home Health RN. The Home Health RN will be responsible for providing nursing care to patients in their homes, ensuring that they receive the highest quality of care and support. RNs will be responsible for assessing the patient and determining a Plan of Care for the physician to review, recertification and/or discharging.

We are looking to hire in all of California. Immediate need for Los Angeles, Fresno, Clovis and McFarland.

Responsibilities:

-Performing initial assessment (OASIS) and creating the Plan of Care

- Re-evaluates patient's nursing needs on a regular basis.

- Monitor patients' progress and make necessary adjustments to the care plan

- Communicate with patients, families, and healthcare team members to coordinate care and ensure that patients' needs are met

- Educate patients and families on disease management, medication management, and proper nutrition

- Document all patient care activities accurately and timely

- Participate in quality improvement activities and maintain compliance with state and federal regulations

Requirements:

- Active Registered Nurse (RN) license in the state of practice

- Experience in home health, hospice, or a related field is preferred

- Strong clinical assessment and documentation skills

- Excellent communication and interpersonal skills

- Dependable, responsible, and able to work independently

- Valid driver's license and reliable transportation

*Additional skills such as wound care, blood draw, IV infusion, etc. are a plus and you will be placed with patients who need that care and the pay will be adjusted accordingly.
