Salary and Benefits

• The pay range for this position is $62,000 - $79,000
• Shift Differential: $2.75 per hour every day from 7:00 PM to 7:00 AM

Responsibilities

• 9:00 AM - 7:30 PM available
• Service Area: West Palm Beach available
• Service Area: Boynton Beach available
• 12:00 PM – 10:30 PM available
• Service Area: West Palm Beach available
• Service Area: Royal Palm Beach / West Palm Beach / Jupiter available
• Service Area: Boca Raton / Lake Worth available
• Every Other Weekend Required (Replaces Days During Week)
• Carries primary responsibility for evaluation of all patients referred for hospice services.
• Is able to articulate all components of the organization's continuum of care and refer patients and families to those as necessary.
• Assists referring physicians and/or his/her designated liaison with coordinating a plan of care/treatment for hospice appropriate patients and their families.
• Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
• Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.

Requirements

• Clear and active RN license in the state of Florida.
• Adequate and reliable means of transportation with adequate insurance and a valid Florida driver’s license.
• One year in medical/surgical/telemetry or acute care nursing required.
• Hospice nursing experience preferred.
• Bilingual (English/Spanish) preferred.
