Minimum Requirements:

A Master's Degree in a counseling or other related field (Required)

At least one year of experience working with clients with a primary mental health diagnosis

Valid Tennessee Licensure - LPC, LMSW, LCSW (Preferred)

Essential Responsibilities:

1. Completes psychosocial intake on clients within appropriate timeframe.

1. Completes the master treatment plan within appropriate timeframe.

1.  Counsel clients or patients, individually or in group sessions, to assist in overcoming dependencies, adjusting to life, or making change 

1. Collaborate with other staff members to perform clinical assessments or develop treatment plans. 

1. De-escalates ACA and other patient problems. 

1. Conducts and documents a daily groups for all clients.

1. Assist clients with appropriate services in order to coordinate care with patient’s treatment needs, including, but not limited to; phone calls, presence in treatment letters, faxing of documentation, coordination with appropriate entities, as necessary, etc.

1. Documents all activities in the patient medical record and files all notes in the charts on a daily basis for UR purposes. 

1. Maintains inventory of Mental Health and AA/NA literature and patient orientation manual with distribution to all patients. 

1. Counsel family members to assist them in understanding dealing with, or supporting clients. 

2. Attends daily staff meetings to enhance communication with clinical staff regarding program and patient issues. 

3. Participate in required staff training (minimum 20 hours). 

1. Complete all assigned paperwork in a neat, accurate and timely manner. 

1. Complete incident reports according to policy in a timely, accurate manner. 

1. Communicate with other staff as necessary to promote quality patient care. 

1. Participate in maintaining clean, properly supplied and efficient work areas. 

1. Observe and report safety and maintenance problems. 

1. Interact appropriately with patients, showing them respect while maintaining boundaries. 

1. Maintain professional demeanor at all times. 

1. Exercise sound professional judgment and seek assistance as necessary to effectively manage patient behavior.

1. Protect the privacy of all patient information in accordance with TIME privacy policies, procedures and practices, as required by federal and state law, and in accordance with general principle of professionalism as a health care provider. Failure to comply with TIME policies and procedures on patient privacy may result in disciplinary action up to and including termination of employment. 

1. May access protected health information (PHI) and other patent information only to the extent that is necessary to complete your job duties. The incumbent may only share such information with those who have a need to know specific patient information you have in your possession to complete their job responsibilities related to treatment, payment or other TIME operations. 

1. Expected to report, without the threat of retaliation, any concerns regarding TIME policies and procedures on patient privacy and any observed practices in violation of the policy to their supervisor. 

1. Performs other duties assigned.
