We are looking for dedicated and compassionate Registered Nurse to join our Home Care team. The successful candidate will be responsible for providing high quality nursing care to patients in their homes, while also ensuring that all long-term care plans are followed. The ideal candidate will have experience in home health, a compassionate nature, and excellent communication skills. This is an excellent opportunity for a Registered Nurse to join a team of dedicated professionals and make a difference in the lives of our patients.

Responsibilities:

• Provide skilled nursing care to home health patients in accordance with physician orders and agency policies and procedures.
• Assess patient's condition and needs, develop a plan of care, and implement the plan of care.
• Provide patient and family education regarding disease process, medications, nutrition, and other health-related topics.
• Monitor vital signs, administer medications, and provide treatments as ordered by the physician.
• Document all patient care in accordance with agency policies and procedures.

Required Credentials:

• CNA License or be able to successfully pass a PCA Test (we offer training)
• CPR Certified (we offer training)
• First Aid Trained (we offer training)
• Negative TB test
• Clean Criminal Background
• Valid Driver's License
• Valid Auto Insurance
