Thompson House is a small, nonprofit, 43-bed long-term care/skilled nursing facility in Brattleboro, VT. We average 3 to 5 skilled residents at any given time, averaging 1 admission per week and 3 discharges a month (give or take), approximately 35-38 LTC residents at any given time. The primary duties of this position would be working with residents, staff and their families in order to develop and document individualized plans of care. The facility will provide extensive training on MDS assessment completion and submission and will fund certification courses.

Responsibilities:

- MDS Assessment Completion for a 43-bed LTC/SNF.

-Care Coordination Meetings and Care Plan Updates

- Assign accurate ICD-10 codes for proper billing and reimbursement purposes

- Coordinate with interdisciplinary teams to ensure accurate and timely completion of MDS assessments

- Monitor Medicare regulations and guidelines related to MDS assessments and make necessary updates to ensure compliance

Qualifications:

- Valid RN license in the state of practice

- Minimum of 2 years of experience in acute care or long-term care setting

-Knowledge of Medicare regulations and guidelines for MDS assessments

- Proficiency in ICD-10 coding and medical terminology

- Familiarity with electronic health record systems, Point Click Care

The ideal candidate would have experience with PCC Careplanning, careplan meetings, and MDS completion.

Pay: $39.00 - $46.00 per hour
