Description

PURPOSE:

The Wraparound Facilitator’s primary purpose is to provide empowerment, guidance, support, education and care coordination to the youth and families in their care. Performance will be measured by your individual outcomes, achieving your individual targets/goals, your contribution to your overall program team/department and your buy-in to the culture of the organization.

Requirements

POSITION SPECIFICATIONS AND EDUCATION:

Must have a valid Driver License and meet any credentialing, licensing, and privileging standards as it pertains to the department you are in. 

Education requirement for this individual contributor role is: Bachelor’s Degree in human services or related field. 

A minimum of 2 years of relevant professional experience 

Proficient in Microsoft Office Suite application software, excellent written and oral communication skills. Must meet designation as a Qualified Professional in NC.
