Masters in Social Work from a School of Social Work accredited by the Council on Social Work Education or an equivalent degree as determined by the Council on Social Work Education; or PhD/PsyD in Psychology at an accredited university or college. Licensed Clinical Social Worker (LCSW) or Licensed Clinical Psychologist (PhD; PsyD). Minimum one year of experience in a behavioral health setting.

The Einstein Philadelphia Family Medicine Residency Behavioral Health Consultant (BHC) is a member of the Family Medicine at Logan Plaza care team providing care for a diverse patient population seeking or receiving careat Einstein Family Medicine at Logan Plaza.. The BHC improves the identification, treatment, and management of psychosocial and behavioral challenges and medical conditions by As the clinic is the foundation of the family medicine residency, the BHC will be involved in resident education- including didactics, precepting, and observation. The BHC is expected to demonstrate competency in the validated and evidence-based methods of assessment and intervention that align with the principles of trauma-informed care and brief treatment to promote recovery and resilience. The BHC is also required to demonstrate a proactive, team-oriented work ethic, whereby they collaborate in real-time with medical providers (both attendings and residents) and clinical staff. Responsibilities include:

  • Screening patients for mental health and substance use disorders using evidence-based screening tools;
  • Providing comprehensive assessment and diagnosis of behavioral health disorders;
  • Evaluating crisis situations and applying appropriate interventions;
  • Providing targeted, immediate, brief intervention for various behavioral health needs, including but not limited to behavioral interventions for chronic medical conditions, anxiety disorders, depressive disorders, mood disorders, substance abuse and dependence, pediatric behavioral concerns, and pediatric developmental concerns
  • Addressing mental or emotional barriers to plan of care adherence;
  • Employing problem-solving techniques to uncover a patientrsquo;s internal motivation and support them in overcoming identified barriers and achieving identified goals;
  • Offering consultation to medical providers
  • Assisting with identifying and processing appropriate referrals to community resources
  • Developing and maintaining quality metrics, in collaboration with Einstein Community Health Associates (ECHA) and organizational leadership;
  • Participating in quality improvement activities;
  • Assisting attending and resident clinicians in developing care management processes, such as the use of guidelines, disease management techniques, case management and patient education to improve self-management of chronic disease;
  • Holding their own sessions for behavioral therapies and interventions;
  • Coordinating professional development opportunities for staff to learn more about behavioral health methods;
  • Completing patient records and documentation in the prescribed format and communicating patient progress to their primary care providers;
  • Tracking patient follow-up and clinical outcomes using a registry. Documenting in-person and telephone encounters in the registry and using the system to identify and re-engage patients.
  • Collection of and/or supervises all data collection provided to the internal organization, government, or other agencies
  • Facilitates social work and/or case management activities for patients of the clinic
  • Facilitates completion of CY-47 or Act 70 reporting (Child Abuse/Elder Abuse) in cooperation with office physicians

The ideal candidate will demonstrate flexibility and creativity, clinical expertise, initiative, leadership, and communication skills necessary to collaborate with a medical team in a fast-paced clinical setting. The ideal candidate will also have an interest in graduate medical education. The ideal candidate will have experience and the desire to work with diverse communities, including LGBTQ communities, patients with English as a second language, patients with varied religious backgrounds, patients with disabilities, patients with substance use disorders, etc.

This position requires providing service to the following age population(s): children, adolescents, adults and older adults in a manner that demonstrates an understanding of the functional/developmental age of the individual served.
