Our services include waiver case management, community living services, care coordination, respite care, and community-based residential programs.

Program Director – LTC and DD Waiver Case Management

FLSA Status: Full-time, Salary, Exempt
FTE: 1.0
Hiring Range: $74,500 - $79,040 annually

Position Overview

The Program Director is primarily responsible for the overall planning, quality control, regulatory compliance and general oversight of our LTC Anoka and Hennepin DD teams. In this role, you will use your leadership skills to mentor and develop Program Managers, Case Managers, and Case Aides. You will also play a crucial role in overseeing the financial well-being of the division and partnering with lead agencies to identify opportunities to improve and expand the services we provide.

Key Responsibilities

  • Foster a culture of transparency through open and frequent communication. 
  • Provide support and development of Program Managers through one-on-one and team meetings.
  • Serve as the primary point of contact for lead agencies and ensure services delivered meet contractual obligations and maintain compliance with the laws and regulations that govern our work.
  • Ensure transparency and equity in accountability for all employees on your teams. 
  • Recruit, orient, and train Program Managers, Case Managers and other employees as needed.
  • Review DHS informational and instructional bulletins relevant to case management services and ensure that case managers receive updated materials and guidance in the provisions of case management services as well as trends in case management.
  • Ensure all programmatic services provided to individuals in assigned programs are of high quality and are consistent with the person-centered philosophy and practice of Thomas Allen.
  • Monitor trends in revenue and expenses. Ensure that data is accurate and partner with Senior Director to make business decisions accordingly.
  • When interacting with individuals served and their care teams, foster trusting relationships, emphasizing empathy, effective communication, and compassion.
  • Lead and inspire a team of dedicated professionals to provide the highest quality care and support to individuals with disabilities

Required Qualifications

This position is required to meet the standard for Qualified Developmental Disability Professional (QDDP) as defined by theDepartment of Human Services

  • A bachelor's degree (at minimum) in social work, special education, psychology, nursing, human services or other fields related to the education or treatment of people with developmental disabilities or related conditions.
  • Must have at least three years of professional experience working with persons with a developmental disability or other vulnerable populations.
  • Must have at least two years of supervisory experience.

Preferred Qualifications

  • Experience in CADI, BI, EW, DD waivers, and the AC program.
  • Knowledge of MA, Medicare, and MN Healthcare programs.
  • Familiarity with community resources and providers.
  • Strong organizational skills and attention to detail.
  • Proficiency in computer basics and familiarity with the Microsoft Office suite.
  • Certified secondary language proficiency (Increased rate of pay).

Additional Requirements

  • Hold a valid driver's license, reliable vehicle for work purposes, acceptable driving record, and proof of auto insurance.
  • Demonstrate excellent written and verbal communication skills in English.
  • Pass DHS required background study
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
