Position: Occupational Therapist - Home Health Care

Responsibilities:
- Conduct comprehensive assessments to evaluate clients' functional abilities and limitations.
- Develop individualized treatment plans to help clients achieve their goals for independent living and optimal functioning.
- Provide skilled occupational therapy services to clients in their homes, including therapeutic exercises, adaptive equipment training, and environmental modifications.
- Monitor clients' progress and adjust treatment plans as needed to ensure optimal outcomes.
- Collaborate with interdisciplinary healthcare team members to coordinate care and maximize clients' independence and quality of life.
Requirements:
- Valid license to practice Occupational Therapy in the state of Virginia.
- Previous experience in home health care or geriatric care preferred.
- Strong clinical assessment, communication, and interpersonal skills.
- Compassionate and client-centered approach to care.

- CHAPS accreditation knowledge or experience is a plus.
