Job Description:

Starting annual salary starts at $18,000 commensurate with experience

Urgent Crisis Center

The Urgent Crisis Center (UCC) is a walk-in, police or ambulance psychiatric crisis stabilization program designed to triage, stabilize, and assess youth who are experiencing psychiatric crisis, and then link the youth/family to a lower level of care within the community. This is a fast-paced environment that operates 24/7, 365 days of the year. As part of an interdisciplinary team, the Family Navigator will support and guide the family as they work through their child’s mental health crisis.

JOB SUMMARY

The Family Navigators will advocate, mentor, engage, empower, and support families to take the lead in their own Plan of Care and helps connect families to needed resources within their own communities.

KEY RESPONSIBILITIES

  • Identifies and utilizes community resources and supports to meet the needs of the family.
  • Refers and connects families to support programs and services, so families reach their optimal functioning.
  • Empowers families through education and support to enable them to take the lead role in planning for and responding to their individual family’s needs.
  • Role models and provides feedback to cultivate nurturing relationships with families and develop connections for families to access resources in the community.
  • Works as a team member with other program staff to respond to family and programmatic needs.
  • Develops and maintains collaborative relationships with other community agencies and organizations in consultation with supervisor, to provide comprehensive and effective treatment services for families (i.e., schools, courts, hospitals, etc.).
  • Accurate and timely completion of documentation and reports, and any other documentation and reports required by agency and program policies and funding sources.
  • Partners with supervisor to identify areas and develop plans for professional growth.
  • Contributes to agency visibility and development of community linkages by representing The Village in community groups or committees, as assigned by direct supervisor.
Required Experience:


EDUCATION

  • High school diploma with experience in family skills and community services required. Bachelor’s degree in social work or an equivalent human services field preferred.

EXPERIENCE

  • Bilingual - Spanish preferred
  • Strong communication skills both verbal and written.
  • Solid general computer skills such as typing and navigating websites is required.
  • Physically able to perform the essential functions of the position, with or without reasonable accommodations.
