The Tandym Group is Hiring a Social Worker (LMSW / LCSW) - Case Management Near White Plains, NY
Responsibilities:
The Social Worker will:
- Complete social work comprehensive assessment for all patients referred or in need of social services
- Comply with social work documentation and its requirements
- Provide psychosocial information for patients transferring to other units to ensure continuity of care
- Provide supportive counseling, crisis intervention, and grief counseling to patients and their families/ significant others, as appropriate
- Assist patients with adjustment to treatment, including problems of depression, anxiety, denial and/ or anger
- Provide information, referral and advocacy to community agencies.
- Advocate and serve as a liaison for patients with community agencies
- Coordinate individual interdisciplinary case conferencem as needed
- Lead education and support groups for patients and/or families/significant others, as appropriate
- Identify, evaluate and design proper safety plans for victims of crimes/abuse
- domestic violence, rape and sexual assault.
- Maintain and complete statistics of case activities as outlined in the departmental policy
- Perform other duties, as needed
- 2 years of experience in an Acute Care setting
- Master’s Degree in Social Work
- NYS License Master Social Worker (LMSW) and/or License Clinical Social Worker (LCSW)
