A New York City-based healthcare network is currently seeking an experienced, licensed Registered Nurse (RN) to join their Home Care team in Brooklyn as a Nurse Team Leader. In this role, the Nurse Team Leader (RN) will be primarily responsible for coordinating, implementing and continually evaluating the home care needs of patients.
Responsibilities:
The Nurse Team Leader (RN) will:

• Precept new Nurses and for supervise peers on their field visits
• Review patient insurance information
• Coordinate activities involved in each individual care plan
• Make sure that pertinent findings are shared with family and professional caregivers
• Perform other duties, as needed

Qualifications

• 2 years of previous Home Care experience
• Bachelor's Degree in Nursing
• NYS Registered Nurse (RN) license
• Solid assessment and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

Desired Skills:

• OASIS certification
• Previous exeprience in a Supervisory role
