The Tandym Group is Hiring a Nurse Assessor (RN) Near Long Island, NY

A home healthcare organization in New York City is currently seeking a new, licensed Registered Nurse (RN) to join their team in Queens as their new Nurse Assessor. In this role, the Nurse Assessor (RN) will be responsible for functioning as the medical / physical health lead on the Housing Provider Team responsible for assessing class members’ service needs currently and upon transition to their supported apartments. 

Responsibilities:

The Nurse Assessor (RN) will:
  • Formulate specific clinical summary documentation and recommendations about class members’ transition to their apartments
  • Provide professional expertise necessary to determine class members' eligibility
  • Perform health assessments based on reports from a doctor and records and medical provider by approving or denying services eligibility
  • Make recommendations related to the care needs regarding the class member’s health and safety welfare
  • Formulate specific clinical summary documentation and recommendations about class members’ transition to their apartments
  • Maintain up-to-date charting and other documentation as required; perform periodic home visits on class members to assess the client’s functioning in the community
  • Provide crisis intervention, including emergency hospital admission support
  • Coordinate medical/medication services with external providers, including primary care and behavioral health clinics, Doctors, Psychiatrist, and pharmacies
  • Maintain scheduled assessment appointments booked with class members within a certain number of days from an In-Reach contact with them
  • Perform other duties, as needed

Qualifications:

  • Bachelor's Degree in Nursing
  • NYS Registered Nurse (RN)
  • Previous experience in Primary and Behavioral Health Intake Assessment.
  • Knowledge of the Community Mental Health System and various community resources and understanding of entitlements and enrollment requirements related to Managed Long-Term Care and Certified Health Home agency
  • Microsoft Office Suite proficient
  • Solid assessment and documentation skills
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • 5 of experience working with older Adults with Serious Mental Illnesses
  • Certification in Geriatric Nursing
  • Proficiency in Health-related computer applications.
