A health services network in New York City is currently seeking a licensed Family Nurse Practitioner (FNP) to join their growing Outpatient team in Brooklyn as their new Lead Nurse Practitioner.
 
About the Opportunity:

• Schedule: Thursday to Saturday
• Hours: 8am to 8pm
• Specialty: Urgent Care and/or Primary Care
• Patient Population: Adults (occasional Pediatrics)
• Patient Volume: 25 to 35 per day (2-3 patients per hour) 

Responsibilities:
The Lead Nurse Practitioner (FNP) will:

• Conduct comprehensive health assessments, diagnose illnesses, and prescribe medications or other treatments
• Manage patients with chronic conditions, provide education on self-management, and ensure continuity of care
• Perform minor procedures within the scope of practice
• Oversee and provide leadership to a team of nurse practitioners, nurses, and support staff in the outpatient clinic
• Implement and oversee quality improvement initiatives to enhance patient care and operational efficiency
• Coordinate and manage provider schedules to ensure appropriate coverage and efficient patient flow
• Ensure accurate and timely documentation of patient encounters and other relevant information
• Provide patients and their families with education on health conditions, treatment plans, and preventive care
• Perform other duties, as needed

Qualifications:

• NYS Nurse Practitioner (NP) license
• Family Nurse Practitioner (FNP) license
• Primary care and/or Urgent Care background

Desired Skills:

• Bilingual (English and Haitian Creole or Spanish)
