The Tandym Group is Hiring a Hospital Liaison (RN) Near New, NY

A healthcare organization in New York is currently seeking an experienced, licensed Registered Nurse (NR) to join their team in Rockland County as a Hospital Liaison. In this role, the Hospital Liaison (RN) will be responsible for developing a hospice presence in the hospital, attend and facilitate rounds and care conferences, confer with all health care providers for the primary purpose of increasing admissions.

Responsibilities:
The Hospital Liaison (RN) will:
  • Develop and maintain positive relationships with referral sources
  • Encourage timely consideration by facility staff and Physicians for the facility as an appropriate mode of care, assisting in identification of eligible clients, and attend relevant meetings
  • Meet with patients and families to assess eligibility, identify needs, explain hospice services and associated benefits, and arrange for and/or conduct hospice admission in absence of the Admission Nurse
  • Serve as covering visit nurse for hospitalized patients in absence of regular staff
  • Work in conjunction with Hospital Case Managers in reviewing the hospital daily census to identify appropriate patients, providing consultation and follow ups requested
  • Identify and develop new opportunities for increasing referrals
  • Identifies hospitalized nursing home residents who meet eligibility criteria and arrange for or conduct admission to facility prior to discharge
  • Participate in QAPI program and hospice sponsored in-service training in compliance with professional, regulatory and agency standards
  • Promote exemplary customer service to Physicians, facility staff, patients, and families
  • Participate in on-call rotation to handle new referrals when office is close
  • Perform other duties, as needed

Qualifications:

  • 5 years of related Nursing experience; 2 years of Hospice experience; 1 year of Acute Care experience
  • Graduated of an accredited School of Nursing
  • NYS Registered Nurse (RN) license
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communicaiton skills (written and verbal)
  • Strong atteniton to detail
  • Highly organized
Desired Skills:
  • Bachelor's Degree in Nursing
  • CHPN
