The Tandym Group is Hiring a Community Health Nurse (RN) Near White Plains, NY

A healthcare organization in New York City is looking to add a licensed Registered Nurse (RN) to join their growing Community Health team in Queens. In this role, the Community Health Nurse (RN) will be responsible for providing skilled care and routine clinical nursing assessment visits to members and families utilizing all elements of the nursing process and in compliance with applicable laws, regulations and organizational policies.
About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 8:30am to 5pm (1-hour break unpaid)
  • Patient Volume: 4 to 6 a day

Responsibilities:

The Community Health Nurse (RN) will:
  • Assess and evaluate the healthcare needs of patients and families with consideration regarding physiological, psychological, social and environmental factors
  • Identify complete, accurate and logical Uniform Assessment System (UAS) for NY and appropriate sequencing of diagnoses
  • Implement the nursing care plan and revises it whenever necessary by regularly assessing, observing, and evaluating the patient’s condition, needs and response to care and makes appropriate nursing judgments and decisions for care plan revision
  • Initiate and apply appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques
  • Administer medications and treatments, as prescribed
  • Teach the patient and family/caregivers self-techniques whenever appropriate and provide instruction regarding medication, diet, safety and treatment modalities
  • Complete, maintain and submit all required documentation that is timely (in compliance with organizational policy) accurate, and relevant
  • Perform other duties, as needed

Qualifications:

  • 1 year of Clinical Nursing experience in a Certified Home Health Agency (CHHA), Acute Care, Medical-Surgical, and/or Critical Care role/setting
  • Graduate of an accredited School of Nursing
  • NYS Registered Nurse (RN) license
  • Computer savvy
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills:

  • Bachelor's Degree in Nursing
