The Tandym Group is Hiring a Care Delivery Field Supervisor (RN) - UAS Near Bronx, NY

A health services network in New York City is currently seeking an experienced, licensed Registered Nurse (RN) to join their team in the Bronx as their new Care Delivery Field Supervisor - UAS.

Responsibilities:

The Care Delivery Field Supervisor (RN) - UAS will:
  • Manage and oversee a team of healthcare providers
  • Conduct initial and ongoing patient assessments to determine their healthcare needs, develop care plans, and ensure that care is tailored to each patient's unique requirements
  • Develop and update individualized care plans for patients, taking into account their medical conditions, treatment goals, and personal preferences
  • Quality Assurance: Monitor and ensure the quality and safety of care provided to patients
  • Educate patients and their families about their medical conditions, care plans, medications, and self-management strategies
  • Oversee the administration of medications and ensure that it is done accurately and in accordance with prescribed guidelines
  • Maintain accurate and up-to-date patient records, including care plans, assessments, progress notes, and other essential documentation.
  • Maintain clear and open communication with patients, families, healthcare providers, and other stakeholders to keep everyone informed about the patient's progress and care plan
  • Continuously monitor and follow up with patients to evaluate the effectiveness of their care plans and make necessary adjustments
  • Perform other duties, as needed

Qualifications:

  • 1 of Home Care experience as a Community Health Nurse
  • Graduate of an accredited School of Nursing
  • NYS Registered Nurse (RN) license
  • Previous experience in a Certified Home Health Agency, Long Term Home Health Care Program, and/or Licensed Agency
  • Solid assessment and documentation skills
  • Computer savvy
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills:

  • Experience in a PACE Program or MLTC
