The Summit Center is Hiring a Coordinator of Adult Day Services Near Amherst, NY

JOB SUMMARY: The Coordinator of Adult Day Services ensures that individuals served in programs and services are provided personal choice and are encouraged and supported in making their own decisions to the greatest extent possible. The Coordinator of Adult Day services ensures that individuals are offered opportunities to participate in their own or greater community to the greatest extent possible in the most integrated setting appropriate to their needs. The Coordinator of Adult Day Services manages their assigned Community Division program(s) including review of admissions materials to determine funding appropriate to referred individuals, authorizing services and managing allocations. This includes the selection, supervision and training of supervisory and direct support professionals (DSP’s), fiscal and clinical accountability, ensuring program files meet quality assurance standards and compliance with state and county laws and regulations. Ensuring the service objectives are met for Office for Persons with Developmental Disabilities (OPWDD) and any other agencies that provide oversight. The Coordinator of Community Services may also fill in and serve as a direct support professional when necessary or as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES: The Coordinator of Community Services must be able to consistently and successfully perform all essential functions of the position.

  1. Ensure that individuals are offered personal choice and are supported in making their own decisions to the greatest extent possible.
  2. Ensure that individuals participate in their community to the greatest extent possible in the most integrated setting appropriate to their needs.
  3. Ensure the safety of all individuals during the provision of services.
  4. Maintain the rights of individuals being served.
  5. Administer all processes relating to assigned programs
  6. Manage funding and departmental expenses for assigned Community Services programs
  7. Provide selection, supervision and training of staff and volunteer
  8. Pursue, recommend and develop the expansion of existing and new programs and services.
  9. Ensure regular communication with individuals (and/or their families) enrolled in programs
  10. Plan, conduct and attend regularly scheduled staff meetings
  11. Assist with a variety of administrative details such as transmitting information, processing bills and invoices, maintaining adequate records, and completing necessary reports
  12. Ensure timely and accurate completion of incident, injury reports or other reports
  13. Share in 24-hour emergency on-call phone rotation as needed
  14. Adheres to The Summit Center Core Agency Values of Adaptability, Accountability, Collaboration, Integrity, Optimism, and Respect
  15. Adheres to the Summit Center employment policies as outlined in the Employee Handbook including, but not limited to dress code, attendance and punctuality, and Paid Time Off
  16. Perform any other duties as requested by your supervisor

QUALIFICATIONS:  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of position skill and/or ability required.

  • EDUCATION and/or EXPERIENCE: M.S. Degree in Human Service Field or related field AND two years of experience in the field of developmental disabilities, OR B.S. Degree in Human Services field AND four (4) years of experience in the field of developmental disabilities. Depending on assignment, may be required to meet the qualifications of a Qualified Intellectual Disabilities Professional (QIDP): (1) At least one year of experience working directly with persons with developmental disabilities; and (2) is one of the following: (i) a doctor of medicine or osteopathy (ii) a registered nurse (iii) an individual who holds at least a Bachelor’s Degree in a professional category

CERTIFICATES, LICENSES, REGISTRATIONS: A valid NYS Driver’s License and Agency approval of driving record according to agency policy and procedures.
