POSITION SUMMARY: Qualified Medication Aide (QMA) provides medication management and assistance with the activities of daily living for assigned residents under the supervision of the Director of Nursing Services. The position is specific to retirement communities in Indiana.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

1. Represents the campus in a positive manner while creating an environment that fosters respect and courtesy for residents and employees alike. Works within the campus programs to promote safety and the well-being of all residents and employees.

2. Upholds the Company’s Mission Statement.

3. Maintains HIPAA standards and regards all medical or healthcare information pertaining to residents and employees as confidential.

4. Administers regularly prescribed medications at the appropriate times for assigned unit during shift.

5. Document in the resident’s clinical record all medications that were personally administered by the QMA.

6. Count, administer, and document controlled substances.

7. Administer previously ordered pro re nata (PRN) medication only if authorization is obtained by the community’s licensed nurse on duty or on call. If authorization is obtained, the QMA must do the following:

  • Document in the resident record symptoms indicating the need for the medication and the time the symptoms occurred.
  • Document in the resident record that the community’s licensed nurse was contacted, symptoms were described, and permission was granted to administer medication, including time of contact.
  • Obtain permission to administer the medication each time the symptoms occur in the resident.
  • Ensure that the resident’s record is co-signed by the licensed nurse who gave permission by the end of the nurse’s shift or, if the nurse was on call, by the end of the nurse’s next tour of duty.

8. Measure and document vital signs prior to administration of medication that could affect or change the vital signs of the resident.

9. Transfers all new prescriptions to the appropriate clinical record from the physician’s orders or admission orders. Faxes all prescription orders to the pharmacy at the time of resident admission.

10. Observes and reports and reactions and side effects exhibited by a resident to the appropriate member of the nursing team.

12. Assists care plan team with care plan assessments as needed.

13. Assists resident with the activities of daily living which include but are not limited to personal hygiene, social and wellness activities, dining, safety and evacuations, bathing, dressing, and transporting.

14. Provides assistance to residents in the dining room including serving, bussing, order taking, and the refilling of beverage containers.

15. Observes residents and notes in the proper chart any changes, unusual occurrences or reactions to medications.

16. Acknowledges and follows the Resident Bill of Rights.

17. Changes bed linens for residents and cleans resident rooms and bathrooms.

18. Assists with resident social activities as needed.

19. Confirms doctor appointments for residents and makes reservations for transportation.

20. Leads (when needed) and participates in daily activity program with residents, resident families and other staff members.

OTHER DUTIES AND RESPONSIBILITIES:

1. Occasional travel may be required for training sessions, continuing education opportunities, emergency situations, and other Company functions.

2. Responsible for knowledge of fire and safety procedures including those involving blood-borne pathogens.

3. Remains in proper attire at all times and maintains a high standard of appearance.

4. Maintains professional development by attending in-service and continuing education programs.

5. Trains in elopement procedures.

6. Performs other duties as assigned by the Director of Nursing Services.

PREREQUISITES:

A. Education:

  • High school graduation or GED required.
  • Current and valid certification as a Nursing Assistant required.
  • Current certification as a Qualified Medication Assistant required.
  • At least 21 years of age.

B. Direct Previous Experience:

  • Two years experience in a similar healthcare position desired.

C. Skills:

  • Ability to work with minimal or close supervision.
  • Must conduct all business with a professional manner and with a high level of confidentiality.

D. Abilities:

  • Work indoors for the majority of the work day (fluorescent lights, HVAC system, carpeted flooring, normal office noise levels).
  • Climb up to two flights of stairs occasionally.
  • Lift objects of 25 pounds or less over the head.
  • Move objects of 40 pounds or less.
  • Remain standing for extended periods.
  • Respond to medical emergencies.
  • Assist with resident evacuations
