The CARE Specialist serves as coordinator of information related to Client Assessment and Referral Program (CARE) Level I assessments and preadmission screens and resident reviews (PASRR) Level II evaluations and is a contact for system coordination between Area Agencies on Aging (AAA), hospitals, nursing facilities/nursing facilities for mental health, KDADS Level II contractor, community mental health centers (CMHCs), community developmental disabilities organization (CDDO), and other community services. The incumbent receives and processes the information for the PASRR Level II evaluations and assists with the CARE screen which are Special Admissions and Level I assessments. In addition, assist in completion of the 3164 requests from KanCare clearinghouse to provide the Level of Care score for nursing facility Medicaid payment process. This position is part of the CARE/PASRR Community Integration team, within the Behavioral Health Services Commission, and provides support for special projects as assigned.
