Position Summary & Responsibilities

Position Summary:

The CARE Specialist serves as coordinator of information related to Client Assessment and Referral Program (CARE) Level I assessments and preadmission screens and resident reviews (PASRR) Level II evaluations and is a contact for system coordination between Area Agencies on Aging (AAA), hospitals, nursing facilities/nursing facilities for mental health, KDADS Level II contractor, community mental health centers (CMHCs), community developmental disabilities organization (CDDO), and other community services. The incumbent receives and processes the information for the PASRR Level II evaluations and assists with the CARE screen which are Special Admissions and Level I assessments. In addition, assist in completion of the 3164 requests from KanCare clearinghouse to provide the Level of Care score for nursing facility Medicaid payment process. This position is part of the CARE/PASRR Community Integration team, within the Behavioral Health Services Commission, and provides support for special projects as assigned.

Job Responsibilities may include but are not limited to the following:

CARE Level II and Resident Reviews:

CARE Level II

  • The incumbent receives the Level I assessment from Level I Assessors with a request/trigger for a Level II.
  • The incumbent reviews all associated documents to determine if the documentation triggers a Level II evaluation and will be sent to the Level II contractor or if the Level II does not qualify and needs to be canceled.
  • If the packet of Level I associated documents meets qualifications for a Level II evaluation, then it will be sent to the Level II contractor to schedule an evaluation.
  • The Level II contractor completes the evaluation and submits the documentation to the KDADS CARE Program/CARE Specialist.
  • The incumbent will send the packet to the CARE II Nurse (or in the absence of the CARE II Nurse, CARE Program Manager) for review and create a determination letter.
  • The CARE II Nurse provides the determination letter to the CARE Specialist for processing and dissemination to the individual evaluated, any legal representatives, and others as appropriate.
  • The CARE Specialist creates an electronic file of all documents related to the Level II evaluation and saves the file on the CARE drive and in Kansas Assessment Management Information System (KAMIS).

Resident Review

  • The incumbent receives the PASRR resident reviews from nursing facilities and once all necessary documentation is received, the incumbent will send to the Level II contractor to schedule a PASRR resident review.
  • The Level II contractor completes the PASRR resident review and submits the documentation to the KDADS CARE Program/CARE Specialist.
  • The incumbent will send the packet to the CARE II Nurse (or in the absence of the CARE II Nurse, CARE Program Manager) for review and create a determination letter.
  • The CARE II Nurse provides the determination letter to the CARE Specialist for processing and dissemination to the individual evaluated, any legal representatives, and others as appropriate.
  • The CARE Specialist creates an electronic file of all documents related to the Level II evaluation and saves the file on the CARE drive and in Kansas Assessment Management Information System (KAMIS).

3164 Requests from KanCare Clearinghouse

The incumbent assists the Program Manager complete the 3164 requests from KanCare as volume of the Level II evaluations allows. This position:

  • opens the CARE Inquiry (3164) and reviews the Request Posted by date and selects the customer with the oldest posted date.
  • then search for the customer in KAMIS and the electronic files to find the PASRR date and Level of Care score.
  • enters the PASRR date and Level of Care score on the 3164 KanCare for the nursing facility payment from Medicaid. The process includes searching for information, keeping track of what step the 3164 is currently in, contacting facilities to obtain missing information in KAMIS. When requested information is obtained and deemed accurate, the completed 3164 is returned to the KanCare Clearinghouse for processing.

    CARE Level I and Special Admissions:

    CARE Level I

  • The incumbent receives the CARE Level I assessment from trained hospital Assessors from around the state of Kansas and some select few from other states on the boarder of Kansas.
  • The incumbent will validate the Level I ensuring all items are completed, signed, and received before entering into the KAMIS system.
  • The incumbent will request missing information via email, phone or fax. Once entered into KAMIS the incumbent will file the file electronically on the KDADS CARE system.

Special Admissions

  • The incumbent will receive special admissions completed by the Kansas Nursing Facilities (NF).
  • Ensure all documents are valid, complete and signed and enter them into the KAMIS system.
  • If more information is needed the incumbent will reach out to the local nursing facilities and request the information needed by email, phone or fax.
  • The incumbent will keep track of Hospital and Nursing Facility Assessors on spreadsheets in the KDADS CARE System.

Qualifications

Minimum Qualifications:

  • One year of experience in general office, clerical and/or administrative support work or education in a relevant field
  • Ability to alphabetize, sort, file, and organize files manually and electronically
  • Proficiency with Microsoft Office programs and ability to use multiple programs at the same time
  • Travel possible

Preferred Qualifications:

  • Experience with customer service
  • Knowledge and/or experience in building collaborative relationships
  • Experience in pursuing needed information through legally acceptable HIPPA and PHI regulation channels
  • Experience with mental health system, diagnosis, treatment processes and medical terminology
