Social Work Case Manager
Job Summary
Responsible for delivering comprehensive social work and medical case management services to patients and families, addressing social determinants of health affecting medical care access. Duties include patient screening, psychosocial assessment, care planning, resource referral, crisis intervention, and counseling. Collaborates with the medical care team, providing consultation on concrete needs and financial services. Facilitates applications for Social Security, Medicaid, Medicare, and Affordable Care Act enrollment. Services cater to various patient populations, addressing routine, specialized, and complex needs.

This position is 100% on-site. Must be able to commute.

Education and/or Experience:
- Master’s Degree in a health-related field required.
- LMSW required.
- MHBE Insurance Assister/Counselor Certification within one year of hire.
- Three years of related experience in a healthcare setting, including community, case management, and outreach.
Major Duties and Responsibilities:
Analytical and Critical Thinking:
- Provides supportive counseling adhering to policy.
- Performs comprehensive intake assessments and documentation duties.
- Executes assigned triage and clinic duties accurately and in a timely manner.
- Offers a wide range of case management and outreach services, including assessments, resource linkage, planning, monitoring, crisis intervention, and supportive counseling.
- Facilitates psycho-educational and social/clinical support groups for targeted patients.
- Demonstrates advanced knowledge of community resources.
Checking, Examining, and Recording:
- Coordinates information, caseload priorities, and referrals.
- Follows up on referrals promptly and accurately.
- Completes referrals in a timely and accurate manner.
- Reviews patient needs, acuities, care plans through periodic case consultations with the supervisor.
Teamwork:
- Collaborates with providers, the healthcare team, and the community.
- Educates providers and the community about medical case management services.
- Participates in interdisciplinary team meetings and staff meetings.
- Provides consultation to facilitate a coordinated continuum of care.
- Ensures informative and well-integrated verbal, nonverbal, and written communications.
- Engages patients and families through outreach and follow-up.
Communication:
- Ensures accessible documentation for the patient's care team.
- Updates care plans addressing medical case management needs.
- Maintains accurate data for reporting.
- Develops relationships and collaborates with community resources.
Skills and Abilities:
- Effective work with patients and families facing social, economic, emotional, behavioral, or health problems.
- Development and initiation of appropriate service plans.
- Provision of services with discretion and confidentiality.
- Establishment and maintenance of effective relationships with staff, agencies, and the public.
