Primary Care Nurse Practitioner
The Staff Pad has partnered with a Recovery-oriented and trauma-informed service provider dedicated to individuals with serious mental illnesses and addictions. Committed to consumer involvement, engagement, and empowerment. The organization aims to enhance trauma-sensitive services.
Responsibilities:
- Provide comprehensive primary care services within the Certified Community Behavioral Healthcare Clinics (CCBHC) model.
- Collaborate with a multidisciplinary team to deliver team-based care, focusing on crisis stabilization, mental health, substance use counseling, and targeted case management.
- Demonstrate a commitment to the principles of Recovery, Consumer-involvement, and Trauma-informed care in both work experience and day-to-day interactions.
- Conduct health and wellness screenings, monitoring, and physical-behavioral health integration.
- Support whole-person health through counseling, skills training, care coordination, health promotion, and education.
- Manage medication and provide psychiatric rehabilitation services.
- Foster preventive approaches to enhance individual health and wellness.
Qualifications:
- Nurse Practitioner certification with a focus on primary care.
- Previous experience showcasing a dedication to Recovery, Consumer-involvement, and Trauma-informed care principles.
- Ability to work within a team-based care model, emphasizing collaboration and holistic health.
- Understanding of Certified Community Behavioral Healthcare Clinics (CCBHC) services and the integration of primary care in behavioral health settings.
