Position Summary:

SCADD is currently seeking 2 compassionate Case Managers at our ERSC (Eastern Region Service Center) program in conjunction with ABH. The Case Manager will provide client placement, referral and discharge planning for individuals regarding behavioral, substance abuse, psychiatric, and/or emotional concerns.

Bi-Lingual a Plus but not required.

About the program:

SCADD partners with the Eastern Region Service Center (ERSC), to provide case management services, and outreach and engagement services to help individuals with substance abuse disorders in Connecticut’s eastern region. The ERSC case managers provide person-centered care and develop recovery plans collaboratively with consumers. They assist consumers with the tools to promote change, enhance well-being, and move toward independent living through gainful employment, housing, and the utilization of social, Twelve Step, and other community supports.

Requirements

Associates degree preferred, or knowledge and experience in Substance abuse and community supports services. Valid CT Driver’s license required.
