The Shire Senior Living is Hiring an Admissions Coordinator Near Irondequoit, NY

Job Summary: The individual holding this role is primarily responsible for growing and stabilizing the census and payer mix of the facilities through a variety of strategies including: exercising a working understanding of the census and payer mix needs of the facilities, using good judgment and positive energy to educate community referral sources about our services, promoting the positive image of the organizations and their business models and ownership structures. In addition to growing and maintaining positive working relationships in the community this role is also responsible to effectively, positively and proactively work with the leadership teams inside each facility to define, establish and maintain productive working relationships.

  • Increase number of Referrals to generate admissions
    • Demonstrates clear and working understanding of the admissions criteria and goals of the facility. Demonstrates clear and working understanding of actual vs budget performance for census and payer mix as well as clinical capabilities of each facility.
    • Responsible to identify primary and secondary referral sources for SNF and Assisted living; developing and growing working relationships with referral sources to increase referrals and admissions to each facility

    1. developing collaborative and cooperative roles both with internal facility leadership and external referral sources specific to admissions process and outcomes

    • Assure seamless and customer friendly care transitions between referral sources and facilities as well as between facilities. Set expectations for admissions with prospective residents and follow through to assure expectations are met or exceeded through an effective
    • Develop routine outreach schedule for consistent message and familiarity of name in the community and to assure potential client base is educated as to their choices for post hospital care and services.
    • Implement and manage referral source satisfaction survey process to facilitate open lines of communication and assure facilities are the preferred choice for case managers and referral sources.
    • Promotes company philosophy within our community and with surrounding areas
    • Promote facility ownership structure, mission, vision and values
    • Show a consistent positive image at local events regarding senior groups that have the potential to be a referral source
    • Educate potential referral sources as to why our model is better than the competition using communication with the team to establish areas of excellence
    • Create in collaboration and cooperation with facility leadership to promote and educate community and professionals
    • Integrate current and contemporary communication mediums and styles to promote the community through a social media presence.
      • Be proficient in digital communication medium, demonstrate effective use of social media platforms for business purposes, work with IT to further develop company websites to include outcomes and performance related data for all potential clients and referral sources
      • Assure facility admissions and marketing material is professional and reflective of company mission, vision and values. Also provide that company marketing material is properly utilized in the community to support business objectives.
      • Operations
        • Work with facility leadership and Medical Management to identify and Quantify effective outcomes and performance metrics to be used in both self/internal evaluation of services as well as define for professional referral sources facility capabilities and relevant outcomes performance statistics. Develop key statistical and trending reporting pertinent to improving referrals and admissions.
        • Identify, track, monitor and trend admission and referral statistics using existing facility tools and software. Specifically maximize the functionality of PCC when it comes to referral tracking and admissions management and resident admission and discharge procedures
        • Make formal reports to facility leadership and ownership regarding census, revenue, payer mix, referral source management, admissions, discharges, etc……comprehensive sales management approach to developing and sustain census and payer mix. Make recommendations to facility leadership and ownership how to improve or enhance admission process; be instrumental in implementing positive change and improving facility performance
        • Develop and implement an effective tracking system for contacting and communication with key referral sources, case managers, and physicians.

        Professional Requirements:

        • Maintains resident confidentiality at all times.
        • Reports to work on time and as scheduled, completes work within designated time.
        • Wears identification while on duty, uses computerized punch time system correctly.
        • Completes inservices and returns in a timely fashion.
        • Attends annual review and department inservices, as scheduled.
        • Attends staff meetings as assigned.
        • Represents the organization in a positive and professional manner.
        • Actively participates in performance improvement and continuous quality improvement (CQI) activities.
        • Complies with all organizational policies regarding ethical business practices.
