Responsibilities
•Provide Nurse Case Managementand Nursing Delegation functions including 45-day on-site assessments,individual assessment of health status, evaluation of medication administrationdelivery system to include: Review of Medication Administration Record(s)(MARs) Client Medications, and Physician Medication Order Form(s) (PMOF’s) forindividuals needing nursing delegation services.
• Perform staff training and retraining as needed/requested
