Description

OVERVIEW:

The Case Manager is responsible intake screening, case management and discharge processing in the Aged Out Foster Youth Transitional Housing program. They will help the participants to achieve stability, increase their levels of income, and help to motivate greater self determination. This includes individual assessments to determine interests and needs and to develop an on-going case plan. The case manager will help link the participant to outside educational/vocational resources in order to reach the goals stated in their individual case plans.

DUTIES AND RESPONSIBILITIES:

  • Participate in the assessment, screenings and referral process for prospective Tenderloin Transitional Housing residents
  • Provide a complete orientation to all new clients assigned to caseload
  • Interview each assigned client during their first week in the housing program to assess the client's history, strengths, academic abilities, vocational abilities, psychosocial development and independent living skills
  • Consult with team members and complete a case plan.
  • Conduct weekly case management sessions with all assigned clients and create written reports detailing client progress and all services utilized
  • Maintain order and cleanliness of the work area
  • Maintain a current list of support agencies and programs in the community
  • Participate in program participants house meetings and communicate any new or revised agency policies. Lead discussions, facilitate problem solving and case planning
  • Attend all staff meetings as required and mandatory in service training
  • Complete accurate and timely client chart maintenance
  • Maintain appropriate social interactions with clients at all times
  • Keep in close contact with Life skills Coordinator and Chemical Dependency Counselor (when relevant) in order to create the most comprehensive and helpful case plan possible
  • Successfully help clients adhere to housing participant agreements
  • Comply with The Salvation Army's accountability policies and procedures
  • Consult and collaborate with other Railton Place staff to ensure continuity of care for clients
  • Interact with clients and intervene in crisis situations
  • Write reports and compile statistical data
  • Other related duties as required

EDUCATION AND EXPERIENCE:

  • High school diploma or equivalent (required)
  • CA driver's license (preferred)
  • BA in Social work or related field or equivalent case management experience
  • Experience working with youth who are in crisis (preferred)

SKILLS AND ABILITIES:

  • Excellent communication skills
  • Excellent computer skills
  • Ability to work effectively with various ethnic and cultural backgrounds
  • Team player
  • Detailed oriented and the ability to work independently with minimal supervision
  • Knowledge of Salvation Amy Programs, practices, polices and organization
  • Knowledge of all funding sources and subsequent client requirements

PHYSICAL REQUIREMENTS:

  • Ability to lift up to 25 lbs. (usually file boxes)
