POSITION DESCRIPTION

POSITION TITLE : GPD-TIP Case Worker

LOCATION / DEPT : Salvation Army Grant and Per Diem (GPD TIP Program Champaign / Danville IL)

REPORTS to (TITLE) : GPD-TIP Program Manager

FLSA CATEGORY : Non-Exempt

STATUS TYPE : RFT

OUTCOMES :

NCI Grant and Per Diem Transition in Place (TIP) program aims to provide community-based case management services using the Critical Time Intervention - Rapid Re-housing (CTI-RRH) model and strength-based case management approach to Veterans and their families enrolled in the Grant and Per Diem Transition in Place (GPD-TIP) program and desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives through this sustainable permanent housing interventions and foster a deeper understanding of the life-changing work carried out by The Salvation Army North and Central Division.

ESSENTIAL DUTIES :

  • Case Management (70%)
  • Provide weekly status updates on referrals, housing inspections, and Veterans served.
  • Participate in housing searches and scheduling housing inspections.
  • Performs monthly health and welfare checks on Veteran occupied rentals.
  • Conduct outreach, intake, and enrollment activities.
  • Meet with enrolled Veterans weekly for Case Management sessions.
  • Conduct Veteran moves into GPD-TIP Rentals.
  • Maintain GPD-TIP Rentals in conditions required per Dept of VA guidelines.
  • Educate Veterans in life skills such as Resiliency, Tenant Education, and Financial Literacy .
  • Facilitate the collection, receipt, and documentation of Veteran Program fees.
  • Connect Veterans with local, community, State, and Federal resources.
  • Advocate for Veteran's need with agencies, Dept of VA, Landlords, and other providers.
  • Aid Veterans in developing skills needed to transition lease into own name.
  • Assist Veterans in filing for SSDI / SSI benefits through SSA's SOAR certification program.
  • Using strengths-based perspective, encourage positive lifestyle changes for Veterans to maintain housing.
  • Assist in providing Veterans with daily supervision through electronic communications.
  • Conduct Veteran Orientation upon program enrollment.
  • Facilitate Veteran transportation as needed.
  • Performs other duties as assigned to the GPD-TIP Program.
  • Provide resources and referrals as needed to Veterans and household members.
  • Flex schedule to meet with full time employed Veterans during non-standard business hours.
  • Administrative Responsibilities (20%)
  • Create Veteran specific goals and track Veteran progress in program.
  • Maintain complete and accurate electronic client records.
  • Provide electronic detailed case note records.
  • Transfer utilities into and out of Program's name.
  • Provide timely rent and utility financial requests.
  • Document all purchases made on Program's behalf in accurate and timely manner.
  • Collaboration (10%)
  • Attend community meetings and Coordinated Entry By-Name-List reviews.
  • Attend community Veteran events such as Stand Down, Veteran resource Fairs and etc.
  • Communicate effectively with Dept of VA and The Salvation Army personnel.
  • Attend weekly and monthly meetings and annual training events.

TRAVEL

  • Involves driving to offsite meetings, conducting Veteran home visits and coordinating Dept of VA GPD Inspections.
  • May be required to attend mandatory meeting events located in Peoria, IL, Bloomington IL, and Champaign IL monthly.
  • Must be willing and able to drive with the service area of Union, Johnson, Jackson, Williamson, Pope, Massac, Saline, Franklin, Pulaski, Perry Hamilton and Gallatin Counties.

EDUCATION / EXPERIENCE

  • Bachelor's degree in social work, public health and / or human services
  • Experience working with the homeless population and / or working with Veterans
  • An in-depth knowledge of VA programs and guidelines
  • And / or Veteran status

POSITION LIMITATIONS (What this position must do)

  • This individual will only commit The Salvation Army resources that have been allocated or approved.
  • This individual will keep the GPD-TIP Program Manager informed on all critical issues relating to their area of responsibility.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.

PHYSICAL DEMANDS / WORK ENVIRONMEN T

  • This position is required to do light physical work.
  • In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and a PC.
  • Adept with Microsoft Office, Word, Outlook, Excel and PowerPoint.
  • Must be able to lift or move up to 30 lbs.
  • The work environment for this position normally includes an office environment with a low to moderate noise level.
  • Must be able to travel to multiple sites.
  • Able to accurately type 45 words per minute.
  • Maintain positive work atmosphere by behaving and communicating in a manner that fosters and maintains positive relationships with co-workers and supervisors.
  • This would include but not be limited to such actions : resolution of conflicts in a positive manner; courteous treatment of staff, clients and visitors;

respect of others' property and person, professional and appropriate communication to and about co-workers, supervisors and subordinates.
