Position Title: Navigation Supervisor

Department: Navigation and Quality

Reports to: Director of Navigation and Quality

FLSA: Exempt

Starting Range: $60,151.19 - $62,608.16 annually

General Description:

The Navigation and Quality Department is made up of specialty teams that work to support people served by The Resource Exchange to ensure appropriate and efficient service delivery. Supervisors within this department are expected to ensure that all responsibilities of the Navigation Department are completed as needed through oversight of staff. Navigation Supervisors cultivate strong business relationships with internal and external community partners to drive business development and positive outcomes for the people served and staff.

Essential Duties/Responsibilities:

  1. Works cooperatively with others, keeps others informed of necessary and factual information. 
  2. Facilitates the operation of a team of which he/she is leading and fosters collaboration across teams.
  3. Works closely with leadership within the Navigation and Quality Department to include the Director, Managers, and other Supervisors. Works closely with leadership to ensure consistency and a positive team environment.
  4. Works closely with leadership and staff from other departments within The Resource Exchange to promote teamwork and efficient interagency communication.
  5. Provide technical assistance and consultation on pertinent areas of expertise to others within the agency.
  6. Progressive and critical thinking to identify future needs and opportunities; identifies problems, obstacles, or opportunities and takes action to address issues.
  7. Demonstrates a willingness to address problems and concerns in a professional and productive manner.
  8. Navigates systemic barriers to service delivery to reduce negative impacts for people served.
  9. Actively participates in efforts to streamline and improve internal processes.
  10. Assists with completion of regular agency performance reviews as conducted and required by Health Care Policy and Financing (HCPF) and Colorado Department of Human Services (CDHS).
  11. Ensures team members complete responsibilities accurately and timely. Team member responsibilities are specific to the position held. Timelines are designated by agency contract, rule and regulation, and TRE policy and procedure.
  12. Ensures that all work completed within the team he/she leads is completed through HIPAA compliant practices.
  1. Provides open and honest feedback to team members. This may include monthly documentation, review of team member’s work, performance reviews, and disciplinary action.
  2. Has a good working knowledge of applicable state and agency data/health record systems. 
  3. Remains well informed and in compliance with rule and regulation, operational and communication memos, waiver amendments, and agency contracts as it pertains to the tasks completed by the team members he/she oversees.
  4. Demonstrates an understanding of the role of Health Care Policy and Financing, governmental entities, and community resources.
  5. Demonstrates an understanding of how team culture impacts interdepartmental relationships and organizational culture both positively and negatively. Works to identify solutions and mitigate negative cultural aspects.
  6. Participates in professional opportunities (committees, workshops, conferences, journal readings) based on Supervisors professional development preferences.
  7. Is able and willing to cross train to learn other functions within the department to assist in completing other tasks when needed.
  8. Initiates, adapts to, explains, and promotes a professional understanding towards operational and systemic changes.
  9. The Supervisor is required to have knowledge, understanding and the ability to train for positions in which they oversee.
  10. Displays an understanding of departmental budget to include speaking to team revenue generation and expenses.
  11. Assists with contract fulfilment through completion of contract deliverables, invoicing and reconciliation of billing on a monthly/quarterly basis as assigned.
  12. Engages in regular review of TRE Policy and Procedure to ensure consistency between rule and regulation, contract, and operational memos.
  13. Completes HR/Business Office designated duties as it pertains to staff employment to include: approving payroll hours, tracking expense invoices to completion and evaluates the cost effectiveness of all overtime requests.
  14. Partners with HR to coordinate FMLA leave, onboarding, performance concerns and termination of staff, and ADA accommodation requests.

Other Duties/Responsibilities:

  1. Completes and documents case management activities.
  2. Represents agency in a professional manner.
  3. Performs driving duties as assigned.
  4. Provides coverage to Navigation and Quality staff as needed. 
  5. Maintains current certification as applicable.
  6. Exhibits professional and safe work practices.
  7. Other duties as assigned.

Supervisory Duties:

  1. Identifies gaps in learning or training needed to help improve the quality of work being performed by staff he/she supervises. Identifies strengths and assists the staff person in capitalizing and sharing those strengths. 
  2. Demonstrates coaching and mentoring by meeting with staff and by attending and observing interactions between them and others.
  3. Assists and delegates training of new staff.
  4. Mentors staff to equip them with the skills necessary to develop leadership and self-directed abilities.
  5. Communicates employee matters to Director of Navigation and Quality.

Job Qualifications:

Education and Experience Requirements:

Minimum:Bachelor’s degree in a human behavioral science or related field of study. If an individual does not meet the educational requirement, considerations may be made if the individual has experience working with long-term services and supports (LTSS) population, in a private or public agency, or has a combination of LTSS experience and education, demonstrating a strong emphasis in a human behavioral science field. Must have at least 1 year of applicable experience. Excellent written and verbal communication skills. Strong advocacy skills. 

Preferred: Working knowledge of Medicaid Waiver programs and requirements; experience working with persons with disabilities. Knowledge of and experience working with community resources and resource development. Extensive knowledge of leadership principles, techniques, practices, and their application to complex casework, group work, and community issues.

Material and Equipment Directly Used: Computers/tablets, Printers, Copy Machines, Personal Vehicle, telephones, Cloud based systems and software, etc.

Working Environment/Physical Activities: Office, community, private residences, commercial properties, and virtual environment settings. Moderate physical activity required, such as walking, sitting, standing, and lifting up to 20 pounds. A working vehicle, valid CO driver’s license (unless active Military family), and minimum insurance levels ($100,000/$300,000/$100,000 liability coverage) are required.
