Some of the duties and responsibilities include:

• Provide one-to-one care with patients in the privacy of their own homes

• Perform duties outlined in client's plan of care

• Follow-up and coordinate case with RN after evaluation

• Observe the patients physical, mental, and emotional conditions

• Maintain a professional nature within patients home​

• Document patient's reaction to treatment on clinical/progress notes, which are written on the day of the visit and incorporated in the patient's record weekly using Kinnser (Wellksky) EMR system

Requirements

• Approved LPN license in the state of Virginia

• Vaccination or valid exemption

• Current CPR license

• TB and Chest X-ray

• ​Prior Home Care experience (preferred)
