The Network
The Primary Health Network is the largest Federally Qualified Healthcare Center (FQHC) in the state of Pennsylvania. We have been serving our communities for 36 years. We currently operate healthcare centers in 16 counties in the state of Pennsylvania and 1 in the state of Ohio. We offer primary care, behavioral health, specialty care (in select counties) and dental services across the state of Pennsylvania.

Position Overview
The Behavioral Health Clinical Care Specialist is an empathic individual with competent observational, clinical, critical thinking, communication, problem-solving and decision-making skills to meet patient needs and contribute to a positive patient care experience.
Location and Hours of Position
The general office hours for this position is 8:30 AM-5:00 PM. The position is located in Altoona, PA.
Education/Experience Requirements
Undergraduate degree in social work or related field, LPN, or RN
Minimum of 2 years of behavioral health clinical experience.
Strong knowledge of behavioral health conditions and strong understanding of behavioral health documentation standards.
Thorough knowledge of Networks policies and procedures.

Duties
Complete thorough and complete assessments of patients entering treatment at PHN. Provides coordinated care to a variety of patients for the network, functions as part of the interdisciplinary care team by providing education and case management directly to patients, and support to Behavioral Health Providers.
Be alert and aware to discuss areas of concern in terms of clinical treatment and documentation on an ongoing basis as requested to the Executive Director of BH.
Competence in conducting impartial investigations pertaining to patient and/or provider needs.
When needed and as appropriate, interview patients to determine their needs and refers them to appropriate community agencies and services (i.e. housing, legal aid, emergency shelter, domestic violence, employment, adult education, food/clothing, etc)
Knowledge of and ability to effectively use and manage the networks information systems including, but not limited to, EMR and word processing as needed.
Ability to work independently with strong sense of focus, task-oriented; Ability to work with culturally-diverse families and communities with the ability to be culturally sensitive, appropriate and non-judgmental
Participates in team meetings within the behavioral health department
Committee involvement as required in performing duties of position.
Will be involved in assisting with event reports and patient complaints as deemed appropriate
Assist with TJC activities
Assist with Quality Improvement activities

The Primary Health Network began as one small community health center site in the Shenango Valley in 1984, with a central focus on providing the highest quality medical care to the people we serve.
