Summary Description:

The Live-In Direct Care Professional (LI/DCP) is a key member in the overall functioning of the Humanity House Program. The LI/DCP will act as the shift lead while on shift and will work in collaboration with the Program Manager to provide total care to individuals living at Humanity House. LI/DCP has a key role in the management of all medical appointments for each individual; oversight of employees while on shift, serving as the overnight on-call and on-call as assigned. The LI/DCP will function as a point of communication between house residents, Price Center leadership, as well as parents and guardians while on shift. The LI/DCP is also responsible for the overall functioning of each shift and ensuring staff participate in and complete their shift duties.

Requirements: MUST be MAP certified and have a valid driver's license with a clean driving record. MUST be willing to live in the house with our individuals. Must be willing to adhere to the City of Brookline policies.

Duties and Responsibilities:

Financial Monitoring and Supervision:

  1. Teach budgeting skills to individuals during outings.
  2. Follow financial plans for individuals to maximize their independence.
  3. Manage individual and house petty cash; obtain correct change and receipts for all purchases to include outing expenses and use of Tax-exempt ID for proper tax deduction.
  4. Responsible for submitting all transactions to the Program Manager for timely documentation.
  5. Responsible for financial oversight during shift.

Medical:

  1. Follow all medical protocols and Doctor Orders; this may include seizure protocols, dietary restrictions, etc.
  2. Respond quickly and appropriately to protocol for all medical emergencies.
  3. Schedule and maintain medical appointments for each individual in the home, under the supervision of the Program Manager.
  4. Transport, accompany, and advocate for residents at their medical appointments, emergency room visits, and urgent care centers as delegated by the Program Manager.
  5. Maintain proper medical documentation and records within the Medical Confidential Binders.
  6. Obtain new and updated Doctor Orders and protocols on an as needed basis.
  7. Obtain Frist Aid and CPR certification within three (3) months of employment and update prior to expiration.
  8. Follow proper PPE guidance as instructed by the Nursing Director.
  9. Maintain MAP certification and obtain re-certification when due.
  10. Administer medications to all individuals in accordance with the MAP Manual.
  11. Maintain and track all medications, under the supervision of the Program Manager.
  12. Participate in shift reports and complete Narcotic count transfers at the beginning and end of each shift.
  13. Complete any medication Leave of Absences (LOA) as assigned by the program manager.

Religious:

  1. Provide opportunities for meaningful, spiritually fulfilling religious activities for each individual according to his/her own religious preference and background.
  2. Support program participants’ right to express their religious affiliations and cultural traditions through preferred and relevant activities. Support diversity of individuals served through respect of religious preferences and/or cultural backgrounds.

Activities of Daily Living; Provide Assistance and Training to Residents in the Following Areas:

  1. Hygiene (shower/bathing, shaving, shampooing, toileting).
  2. Daily dressing and clothes selection.
  3. Laundry and clothing care.
  4. Daily housekeeping chores.
  5. Follow dietary protocols for each individual (nutrition, meal preparation, feeding support, table manners, etc.).
  6. Participate in and implement day-to-day ISP objectives for each individual, as outlined in their ISP.
  7. Responsible for ensuring staff are prompting and participating in all ISP goals and objectives during shift.
  8. Provide support with lifting (two-person physical lift and/ or manual via Hoyer lift); transferring individuals from bed into wheelchair or from wheelchair to bed maybe required.
  9. Responsible for all ADL oversight during shift.

Recreation:

  1. Plan, organize, and conduct diverse educational and recreational activities for all individuals during downtime.
  2. Maximize independence to make recreational activities accessible without a car and without staff as often as possible, if applicable.
  3. Transport individuals to recreational activities, by personal vehicle or agency vehicle.
  4. Provide supervision to individuals in the community, as stated in each individual’s ISP and Price Center Policies.
  5. Responsible for recreation oversight during shift.

Social:

  1. Help residents develop necessary social skills that are appropriate to their life within their community.
  2. Provide simple, clear, nonjudgmental, and supportive counseling and education, regarding sexuality and appropriate expressions of feelings.
  3. Train and encourage regular and socially acceptable use of the telephone and email correspondence, when applicable.
  4. Assist residents in maintaining ongoing family contact to whatever degree is positive for both the resident and his/her family.
  5. Maximize and facilitate peer relationships within the house.
  6. Responsible for social interaction oversight during shift.

Communication:

  1. Ensure each individual has the opportunity for choice (food, TV, activity, etc.) by using their preferred method of communication.
  2. Maintain timely and effective communication regarding individual updates and program needs to the house manager and house assistant manager.
  3. Maintain positive communication with families of individuals, neighbors, employers, and service professionals.
  4. Check Price Center email daily.
  5. Check your personal mailbox located within the residential program on each shift.
  6. Participate in shift reports at the start and end of each shift.
    • Provide a pre/post shift debrief with incoming/outgoing staff regarding any incidents, such as a sick individual, witnessed or unwitnessed fall that requires monitoring, etc.
    • Unexpected hospitalization with detail account.
    • Report any behaviors that occurred on your shift and any other general maintenance issues.
    • Review and sign off on staff communication logbook backdated to last shift worked.
  7. Responsible for communication oversight during shift.

Safety:

  1. Conduct monthly fire drills and follow-up training as needed for individuals who do not meet requirements for self-preservation.
  2. Ensure the residence and egresses are clean and clutter-free. Report all environmental hazards to supervisor.
  3. Ensure staff/ individual ratios are maintained at all times.
  4. Provide proper supervision to individuals at the residence and in the community, as stated in individual ISP’s.
  5. All staff need to be knowledgeable in universal contact, isolation precautions and proper use of personal protective equipment (PPE). Trainings are provided as needed.
  6. Responsible for safety oversight during shift.

Programmatic:

  1. Answer all incoming house phone calls and document messages when applicable.
  2. Assist in routine house and yard maintenance and household cleaning.
  3. Shift Lead will serve as the primary person giving direction and re-training as needed, to regular and relief staff during shifts and as assigned.
  4. Responsible for completing weekly program inventory of supplies for management.
  5. Live-In position is reasonably estimated to be 40hr week position 5 days a week of Monday through Friday, during hours dependent upon needs of individuals (residents) and will be recorded on timesheets accordingly. Any time over 40 hours in a week must be approved in advance by a supervisor.
  6. In addition to the 40 hour working hours described in IX.e, responsible for being on-site and on-call in Residence when indicated by the supervisor from time-to-time for additional hours that is compensable if engaged in work, generally expected to occur overnight but may be otherwise.

Record Keeping:

  1. Provide careful informative daily log entries to facilitate staff communication and coordination.
  2. Document and track program supplies as requested by supervisor.
  3. Complete agency Incident and Accident/Injury Forms.
  4. Daily documentation in each individual’s binder to include ISP objective documentation, bowel movements, hygiene and body charts, menses chart (where applicable), and others as assigned specific to the individual.
  5. Responsible for ensuring all proper documentation is complete during each shift.

Other:

  1. Attend new hire orientation within 3 months of hire.
  2. Attend and actively participate in monthly staff meetings (see staff meeting/supervision policy attached).
  3. Attend regular supervision sessions with supervisor.
  4. Participate in regular DDS certification/licensing (CARF) visits in preparation and presentation during CARF team visits.
  5. Participate in any shopping responsibilities, as assigned.
  6. Participate in and complete all required trainings.
  7. Complete timesheets for hours worked on daily basis.
