MAJOR PURPOSE:
PRIMARY FUNCTIONS:
- Coordinating interdisciplinary resident care conferences with Social Services staff.
- Coordinates and completes the MDS with appropriate CAA's and trigger worksheets; determine assessment reference dates.
- Reviews the resident's medical history.
- Interview appropriate unit staff.
- Reviews resident care plans using the information available and resident care plan library during the MDS completion window.
- Daily review of rehab software for unscheduled assessments.
- Updates the resident care plan library as necessary.
QUALIFICATIONS:
- RN with experience or LPN with extensive experience in MDS. Must be a registered or Licensed nurse in the state of NY.
- Has a working knowledge of MDS and PPS process.
- Knowledge of Submission Process and Interpretation of Validation Reports.
- MDS certification preferred.
