The Pines at Catskill, a beautiful 120-bed Non-Union facility with excellent Survey results, is seeking an MDS Coordinator to join our awesome team!
The MDS Coordinator will be responsible solely for our LTC population, not the Medicare Part A residents.

MAJOR PURPOSE
:
Responsible for overseeing comprehensive resident care plans through clinical assessment, review of resident's medical history, personal interviews, and completion of MDS.

PRIMARY FUNCTIONS:
  • Coordinating interdisciplinary resident care conferences with Social Services staff.
  • Coordinates and completes the MDS with appropriate CAA's and trigger worksheets; determine assessment reference dates.
  • Reviews the resident's medical history.
  • Interview appropriate unit staff.
  • Reviews resident care plans using the information available and resident care plan library during the MDS completion window.
  • Daily review of rehab software for unscheduled assessments.
  • Updates the resident care plan library as necessary.

QUALIFICATIONS:
  • RN with experience or LPN with extensive experience in MDS. Must be a registered or Licensed nurse in the state of NY.
  • Has a working knowledge of MDS and PPS process.
  • Knowledge of Submission Process and Interpretation of Validation Reports.
  • MDS certification preferred.
