Nursing Functions

• Monitor care processes and systems as assigned (i.e. MARs, 24hr book, Alert Charting system).
• Monitor weight report for timely completion, necessary re-weighs and identify those residents with weight gains/losses that require assessment and intervention.
• Report audit findings to DON daily and as needed.
• Maintain tracking systems for side rails and safety devices.
• Initiate data entry of resident chart in Service Minder.
• Create and maintain Service Plan in collaboration with assessing nurse.
• Participate in service plan management in conjunction with nursing.
• Gather data and document minutes for Quality Council meetings.
• Perform, reconcile and submit end of the month billing from Service Minder to Business Office Manager.
• Maintain schedule of care conferences according to standards.

Job Qualifications

• Must possess the ability to make independent decisions, follow instructions, and accept constructive criticism.
• Must be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
• Must be able to work with ill, disabled, elderly, and emotionally upset people within the facility.
• Must be able to speak, write and understand English in a manner that is sufficient for effective communication with supervisors, employees, residents, and families.

Education and Experience

• Must possess a current and active license to practice as an RN in this state
• Experience in a home care, assisted living, or long-term care facility
