Job Title: Facilitating Case Manager

Reports To: Managing Director

Summary

The Facilitating Case Manager has the primary responsibility of providing case management services for criminal justice impacted individuals. The Facilitating Case Manager will be assigned specific clients, referred to as a 'caseload' and monitor the service delivery for these clients while they are in the program from intake to discharge

Primary Duties and Responsibilities

• Ensures compliance with standard organization, and program contract policies and procedures of their assigned unit.

• Coordinate daily program activities in accordance with the established schedule. These

activities may include Daily Groups and Community meetings; Weekly program curriculum

lectures, Life Skills seminars, Caseload groups, and specialized groups focusing on the areas

of Anger Management, Relapse Prevention, Job Readiness preparation and Parenting skills.

• Conduct initial intake session with new clients in which the case manager will orientate the client about the program requirements

• Conduct Bi-weekly Individual counseling sessions with assigned caseload. These sessions may include the following tasks: completion of a monthly progress report; Monthly Release planning; Goal setting; Crisis Intervention; Behavioral adjustment sessions; Psychological referrals, and Discharge Planning in order to provide recommendations and referral sites for continued programming upon their release.

• Maintain charts (electronic and hard-copy versions) for assigned clients caseload and ensure all documentation is completed in accordance with established deadlines and program policies.

• Provide evidence-based CBT programs and other life/parenting skills programs to participants.

• Conduct group and individual meetings utilizing motivational interviewing techniques.

• Collaborate with participants to develop and implement a case management plan (CMP).

• Facilitates delivery of cognitive behavioral curriculum in individual and group settings focusing on the areas of motivation for change, social adjustment, personal management, social skills development, cognitive restructuring, and development of problem-solving skills.

• Evaluates progress of participants. Conducts a periodic review of curriculum and resources available to enhance participation in post release programs.

• Maintains class rosters, attendance records, and provides completion certificates for evidence based programs and other life skills curriculum as required.

• Supports the assessment of participants' needs and makes referrals to appropriate staff. Aids in planning and developing post-release plans and encourages participants to assume the responsibility for post-release behaviors and activities.

• Participates in orientation of participants regarding all evidence based behavioral treatment programs.

• Facilitates and/or co-facilitates, group counseling from a structured manual.

• Assists in the coordination of participant program schedules to ensure the criminogenic needs of the participant are met.

• Performs other duties as assigned

Pay: $19.37 - $23.33 per hour
