The Mental Health Center is a CARF accredited private, not-for-profit community mental health center providing comprehensive psychiatric outpatient and rehabilitative services to children as young as four, adolescents, adults, and their families.

The mission of The Mental Health Center is to enhance the quality of life and promote recovery for individuals and families by providing the highest standard of person-focused preventative and therapeutic psychiatric care.

DUTIES AND RESPONSIBILITIES:

1. Provides experienced level of services that may include service linkage, assessment and planning of services, patient advocacy, monitoring of service delivery, patient outreach, and documentation.

2. Provides case management services for a caseload of eight to 15 patients.

3. Observe the behavior and physical condition of patient’s to assess mental health status.

4. Will manage and maintain medical records of assigned Mobile Treatment cases while participating on Medical Record Review team.

5. Provides crisis management, family supportive interventions and case management services.

6. Participate in 24-hour emergency coverage rotation and provide crisis intervention services.

7. Provides liaison services for patients with various community agencies.

8. Functions as part of a treatment team to serve and meet the needs of patients.

9. Participates in the development and implementation of patient treatment and recovery goals.

10. Documents all clinical activities in accordance with agency requirements.

11. Works schedule according to needs of agency.

12. Attends committee meetings as assigned.

13. Participates in ongoing clinical supervision according to agency policy.

14. As a community mental health organization, we serve people of all ages and walks of life. Usually our customers are pleasant and agreeable. Sometimes though, due to their illness or other human circumstances, they can be difficult. People who have communicable diseases, poor hygiene, and surly dispositions are entitled to quality service without bias. You may be assigned and expected to work with people who fit these descriptions.

15. Performs other duties as necessary.

MINIMUM QUALIFICATIONS:

--- High school diploma with one year experience.

--- Must possess a valid driver’s license.
