Salary Range: $85K-$105K


Summary Description
:

The Psychiatric Nurse Practitioner (PNP) will work as part of a multidisciplinary team in the Assertive Community Treatment (ACT) Program serving Adults. The PNP will provide direct client care including assessment, medication management, medication assisted treatment (MAT) and psychiatric treatment to individuals experiencing substance use and co-occurring disorders. They will support recovery by providing services that reflect our deeply held values, including the recognition of individual rights of self-determination, choice, shared decision-making, and collaboration. They will utilize and promote evidence-based practices, always embracing a recovery-oriented, trauma-informed, and person-centered approach.


Key Responsibilities
:

  • Conduct substance use and psychiatric assessments, including substance use/psychiatric history, course of illness, response to treatment, mental status examination, and DSM-V diagnoses.
  • Present and discuss assessment/client care at team meetings and case conferences.
  • Prescribe psychotropic medications and conducting regular assessments of therapeutic responses and side effects, including education of clients regarding psychotropic medications.
  • If needed, administer medication- intramuscular injections (IM) according to Rx order(s) as documented in EHR.
  • Assess and discuss with client’s eligibility for MAT.
  • Provide recommendations on level of care and treatment setting to meet client’s individual needs.
  • Receive and review toxicology and lab results, incorporating into electronic health record.
  • Coordinate care to address medical issues in collaboration with other staff.
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures.
  • Collaborate and coordinate with outside care providers and referral sources, as needed.
  • Ensure that prescriber billing is timely and accurate.
  • Provide support, education, and counseling to family members/significant others of clients, engaging them in the treatment/recovery process, to assist in their family member’s treatment and recovery.
  • Provide on-site crisis assessment and management of clients during regular work hours, both on-site and over the phone.
  • Follow the established policies & procedures.
  • Maintain appropriate professional standards.
  • Other related duties.


Key Competencies
:

  • Principles, practices, and methods of substance use disorders treatment including interviewing, assessment, individual and group counseling, and crisis intervention.
  • Applicable city, state and federal guidelines and laws affecting substance use treatment, mental health treatment and services.
  • Characteristics and types of mental and emotional illness.
  • Familiarity with and ability to use an Electronic Health Record system.


Key Performance Indicators (KPIs)
:

  • Ensuring timely completion of accurate client records.
  • Evaluating the effectiveness of crisis management interventions.


Requirements
:

  • Must have a valid Driver’s License and a driving record that is satisfactory to our insurance carrier. The PNP will be using agency vehicles and is responsible for adhering to Agency Driver Policy.
  • Must be a NYS Licensed Psychiatric Nurse Practitioner.
  • Must exhibit patience, flexibility, compassion, and a deep belief in recovery.
  • Must maintain an active license in good standing to practice in New York State.
  • Must maintain an active unrestricted federal and state DEA registrations.
  • Must comply with the standards and requirements of all applicable federal, state, local and other laws, rules, and regulations governing medical services.
  • Must comply with all professional standards and all applicable agency policies and procedures, including the agency’s compliance program and code of conduct.
  • Understanding of 42CFR, HIPAA Privacy Standards and related agency policies.
  • Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
  • Must be and maintain clearance through the NYS Justice Center through fingerprinting for a Criminal History Records search.
  • Must be and maintain clearance by the DCJS New York State Sex Offender Registry.
  • Must be and maintain clearance by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
  • Must be and maintain clearance by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.
  • Must be legally eligible to work in the United States.
