Are you a dedicated and compassionate professional looking to make a positive impact in the lives of children and adolescents? We are currently seeking a full time Care Manager for our Health Homes Serving Children Program.

MHA Westchester's Health Home Serving Children Program is committed to providing community-based care management services to children ranging from birth to 21 years of age. As a Care Manager (CM) on our team, you will play a crucial role in coordinating the development and execution of individualized, community-based plans encompassing social, emotional, and medical services for children and adolescents with special needs.

Required Knowledge and Skills:

1. In-depth knowledge of behavioral health diagnoses and symptomatology.
2. Completion of CANS-NY Training and certification for CANS-NY use.
3. Proficient in effectively engaging clients.
4. Exceptional organizational skills and self-motivation.
5. Familiarity with and ability to use an Electronic Health Record system.

Qualifications:

1. Bachelor’s Degree with two years of relevant experience, or licensed as a Registered Nurse with two years of relevant experience, or a Master's Degree with one year of relevant experience.
2. Basic computer knowledge is required.
3. Reliability and the ability to work independently, with a strong understanding of the importance of maintaining confidentiality.
4. Possession and maintenance of a valid Driver's License with a satisfactory driving record according to our insurance carrier's standards.
5. Maintenance of a current NYS Defensive Driving certification.
6. Clearance by the NYS Justice Center through fingerprinting for a Criminal History Records search.
7. Clearance by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
8. Clearance by the NYS Office of Children and Family Services (OCFS) regarding instances of child abuse and/or neglect.
9. Legal eligibility to work in the United States.
