The CDCA facilitates the assessment process for individuals needing substance abuse treatment services and provides accurate evaluation of the client in order for the Clinical Director to evaluate needs and to develop the initial diagnosis(es). In addition, The CDCA will provide consultation and counseling services for clients with diagnosed substance use disorders. The CDCA will also provide clients and their families with the assistance needed to cope with the client’s diagnosis, recommended treatment and life stressors.
Essential Functions and Duties of CDCA:
1. Facilitates and completes the assessment process for individuals needing behavioral health services.
2. Develops, implements and regularly reviews and updates individualized service plans designed to address and eliminate client barriers and obstacles related to daily living and welfare needs, interpersonal skills, health and vocational needs, employment, social skills, recreational needs and family relationships, as determined during the assessment process and on-going treatment.
3. Provides individual and group counseling therapies and sessions.
4. Facilitates effective communication with clients, families and team members regarding the planning, support and development of the client service package.
5. Identifies, recommends and assists clients in accessing community resources and in acquiring the appropriate medical, social, vocational and employment-related resources needed in order to meet goals outlined in the individualized service plan.
6. Facilitates the client referral process to healthcare professionals and other community organizations, as needed.
7. Collaborates with other treatment professionals in order to promote optimal client health, benefits and supports.
8. Empowers clients to increase pro-social decision-making capacity and overall independence.
9. Reports on any observed changes in the client’s mood or behavior, healthcare-related needs or mental health-related needs.
10. Notifies the Clinical and Medical Teams (if applicable) of any changes in the client’s circumstances and maintains all records as instructed by the company’s policies and procedures.
11. Reports any incidents, complaints or abuse regarding a client, staff member and/or vulnerable adult.
12. Attends and participates in scheduled or required staff meetings, training, peer reviews, workshops and supervision sessions while contributing to the overall effectiveness of the team environment.
13. Completes required documentation no later than 48 hours after the service being provided.
14. Adheres to the company’s personnel policies and procedures, JCAHO, OMHAS, Medicaid and other insurance standards, and fulfills documentation and reporting requirements.
15. Informs supervisor when ill or unable to attend to job duties.
16. Identifies program needs and makes suggestions for improvements to the Clinical Supervisor, as needed.
17. Complies with all company policies and procedures, including but not limited to Compliance, HIPAA and Professional Code of Ethics.
18. Maintains at least their current or the minimum required Ohio certification status as a qualified service provider. All CEU certificates obtained during employment with TLC Recovery must be copied for entry into the personnel file.
19. Other duties as assigned, within scope of practice.

Work Environment:
The work environment is at any approved site of TLC Recovery. This position will require travel within the Cleveland, Akron and Canton area. Must provide your own transportation, and be comfortable transporting clients. This is a contract, 1099 position.
